Summary: Experienced Registered Practical Nurse (RPN) sought for Intake Coordinator role in Sault Ste. Marie, Ontario, to assess patient needs, develop care plans, and coordinate services.
We're looking for a skilled RPN to join our team as an Intake Coordinator. In this role, you'll work with patients and their families to assess care needs, determine eligibility for services, and develop individual care and service plans. You'll also link patients with service providers, coordinate and monitor care plan delivery, and establish a helping relationship with patients and their families.

- Establish a helping relationship with patients and their families and balance patient needs and choices with available resources.
To be successful in this role, you'll need:
- Registered Practical Nurse in good standing with the College of Nurses of Ontario.
- Relevant experience in long term care homes or health care and/or in social services.
- Experience working with seniors and/or physically disabled individuals.
- Understanding of issues and priorities within the health care sector.
- Knowledge of tools, systems, and databases used in health care service delivery and management.
- Practical knowledge and understanding of relevant legislation.
- Effective planning, organizing, and evaluation skills to manage multiple patients and provide information reports.
- Ability to accurately complete required documentation, reports, forms, and recommendations.
- Strong interpersonal and communication skills to work with diverse patient groups.
- Effective listening and facilitation skills to work with patients and their families, caregivers, or substitute decision-makers.
- Ability to communicate using various software programs to clearly articulate patient status.
- Strong collaboration skills to participate in projects and committees within the organization.
- Working knowledge of computer software and Microsoft Office applications.
- A valid driver's license and access to a reliable vehicle.
- Advanced oral and written proficiency in English.
We're looking for someone with:
- Experience working with diverse patient groups, such as multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics.
- Case management experience or recent related community experience.
- Ability to speak French or another second language.
