Occupational Therapist required to provide therapy services to individuals in the community, working collaboratively with healthcare professionals and community services to ensure patient-centered care.
The Occupational Therapist will assess patients' ability to function safely in their home and community environment, develop client-centered plans, and provide education and recommendations to patients and families. The role requires strong communication skills, ability to work independently and collaboratively with a multidisciplinary team, and proficiency in technology for documentation of patient outcomes.
Key Responsibilities:
- Assess patients' ability to function safely in their home and community environment
- Develop client-centered plans with patients and families
- Provide education and recommendations to patients and families
- Assist with funding sources to obtain equipment and home modifications
- Monitor patient outcomes and progress towards therapy goals
- Modify patient care plans as required
- Document patient outcomes and current care plans
- Communicate with funding agencies regarding patient outcomes and status changes
- Manage and maintain caseload of clients
External and Internal Relationships:
- Collaborate with interdisciplinary teams to ensure patient care needs are met
- Work with community services to ensure safe transition to community and patient goals are met
Education, Designations and Experience:
- Masters in Occupational Therapy (or degree prepared occupational therapy education prior to 2008)
- Valid registration with COTO and associated liability insurance
- Membership with the Ontario Society of Occupational Therapists is preferred
- Experience in a healthcare environment (long term care, acute medicine, rehab unit, rehab clinic, community visiting program)
- Demonstrated knowledge of community resources preferred
Skill Requirements:
- Ability to utilize computer software programs on a handheld device
- Proficiency in technology for documentation of patient outcomes
- Strong communication skills, both written and verbal
- Ability to work independently and collaboratively with a multidisciplinary team
- Ability to adjust to working in different social and economic environments
Other:
- Must have personal vehicle and possess current driver's license and proof of vehicle insurance
- Willingness to travel regularly during the day to support sites and attend meetings
- A current and original copy of a satisfactory Criminal Records Check is required
- A Vulnerable Sector Search and/or Child Abuse Registry Check may be required
