Nurse Practitioner Adult Heart, Lung & Vascular Program (Job ID: 4891)

The CV-NP Excellence Fellows will work under the tutelage of experienced Nurse Practitioners and Physicians across several specialties highly relevant to the care of the cardiovascular patient. This position is designed to be a transitional role, whereby the selected NPs develop clinical expertise, specialty knowledge, and self-confidence as they move from a novice to a highly skilled Cardiovascular Nurse Practitioner. NP Fellows benefit from one education day each week, dedicated to working toward course requirements for the Certificate in Hospital and Acute Care, through the Lawrence Bloomberg's Centre for Professional Development. The comprehensive fellowship curriculum, enriched with structured mentorship, has been crafted to provide a solid foundation while instilling advanced competencies required for success in cardiovascular surgery.

The CV-NP Excellence Fellows will work within 4 domains of practice:

Clinical Domain:

  • Demonstrates the NP competencies in providing clinical care to patients and their families as the central part of the NP role. The NP works autonomously to diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals, and perform procedures within the legislated scope of practice and a defined Unity Health Toronto Collaborative Practice Agreement
  • By the end of the fellowship term the CV NP will be able to Lead the interprofessional team to provide clinical care for patients on the Cardiovascular Surgery Service including making independent decisions for patient treatment and discharge, including completion of discharge summaries and transfer of accountability to community care providers

Knowledge Translation:

  • Analyzes, appraises, and synthesizes research and best practices to use as a foundation for enhancing the delivery of quality patient care
  • Uses knowledge translation techniques in innovative ways to put research-based knowledge into daily practice and to support and educate other members of the health care team.
  • Contributes to the dissemination of nursing knowledge through presentations and publications
  • Successfully fulfills coursework and continuously meets requirements for the Certificate in Hospital and Acute Care, through the Lawrence Bloomberg's Centre for Professional Development

Scholarship:

  • Contributes to the culture of discovery at St. Michael's Hospital by advancing and/or appraising nursing research
  • Evaluates outcomes of practice, practice patterns and systems of care against local and national benchmarks to identify variances in practice outcomes and population trends
  • Identifies issues related to clinical practice and patient care and develops innovations to improve care and care delivery for patients, families, and communities
  • Designs, directs, participates in and evaluates quality improvement methodologies to promote safe, timely, effective, efficient, equitable, and patient-centered care

Leadership:

The Fellowship will provide the CV NP with the skills required to assume a leadership role as a member of the CV NP team

  • The NP acts as a transformational leader by using ideals, inspiration and intellectual stimulation to influence the behaviors and attitudes of others towards the goal of excellent patient care
  • Acts as a consultant and resource to members of health care team and external groups in the process of decision-making strategic planning and problem solving by committee involvement, participation in program development and evaluation
  • Is eligible for cross-appointment to an academic institution
  • Is a member in the Nurse Practitioners Association of Ontario (NPAO) and other relevant organizations

Qualifications:

  • Adult or Primary Health Care NP (Registered Nurse in the Extended Class) in good standing with the College of Nurses of Ontario (or eligible for registration) required
  • Master's degree in Nursing with specialized clinical focus in Nurse Practitioner Practice required
  • Current ACLS and BCLS certificate required
  • Enrolment in the Certificate in Hospital & Acute Care for Nurse Practitioners through the Lawrence Bloomberg's Centre for Professional Development required
  • Completion of education to prescribe controlled substances required by the College of Nurses of Ontario preferred
  • CNA Certification in Cardiovascular Nursing preferred
  • Demonstrated experience and leadership in education and research
  • Excellent team-building, negotiation and conflict resolution skills
  • Preference will be given to candidates who demonstrate knowledge and experience in critical care, cardiovascular surgery, and/or cardiology
