About the Role

This is a permanent, full-time position that reports to the Manager of Community Supports and is clinically supervised by the Social Worker III - Interprofessional Practice.

Key Responsibilities
  • Conduct thorough assessments to identify clients' needs and develop individualized treatment plans.
  • Provide direct counseling and support to clients and their families to promote positive outcomes.
  • Collaborate with the inter-disciplinary healthcare team to ensure comprehensive care and effective discharge planning.
  • Participate in the Regional Assessment and Placement Committee to ensure seamless transitions and optimal care.
  • Ensure the safety and well-being of clients and staff within the social work treatment area/department.
Requirements
  • High School Diploma or equivalent and successful completion of a Bachelor of Social Work degree.
  • Preference will be given to individuals with 2 years' experience in healthcare.
  • Current registration with the Newfoundland and Labrador College of Social Workers.
  • Evidence of ongoing professional development.
  • Willingness to travel within the region.
  • Demonstrated effective clinical social work practice.
  • Ability to work with minimal supervision.
  • Knowledge of healthcare policies and trends.
  • Basic computer skills training is preferred.
