Health Careers in Saskatchewan is seeking a skilled Registered Nurse to join our Home Care team. As a Home Care Registered Nurse, you will play a vital role in providing high-quality nursing services to patients in their homes.
Key Responsibilities- Assess patient needs and develop personalized care plans in collaboration with the Home Care Manager and other healthcare professionals.
- Provide nursing services in the home, including wound care, medication management, and education on disease management.
- Collaborate with the multi-disciplinary team to set priorities for care and develop standards for care.
- Participate in educational programs and performance evaluation processes to ensure continuous improvement.
- Promote good public relations and participate in studies and research projects within the Home Care Program.
- Two (2) years recent community nursing experience or a broad generalist background.
- Bachelor of Science in Nursing (BScN) or a nursing diploma.
- Basic Life Support Certificate (BLS) and registration/eligibility with the College of Registered Nurses of Saskatchewan (CRNS).
- Long Term Care Experience is preferred.
