Key Responsibilities- Care Coordination
- Collaborate with interprofessional care teams to coordinate care for clients with mental illness and concurrent disorders.
- Conduct mental health assessments, develop, implement, and maintain comprehensive treatment plans based on client needs and best practices.
- Direct Nursing Care
- Plan and provide direct nursing care to clients in compliance with standards of practice.
- Assess client/family history, conduct physical/psychosocial assessments, and develop a care plan specific for the client.
- Therapeutic Relationships
- Establish, maintain, and enhance therapeutic relationships with clients and their informal support network.
- Engage stakeholders in treatment planning and evaluation, provide support and follow-up planning, and promote self-care.
- Treatment Services
- Provide treatment services, including crisis intervention, individual counseling, and education, according to various theoretical models.
- Assess situation, elicit concerns, explore emotional response to illness, identify goals and expectations, and review client and family learning needs.
- Case Conferences and Collaboration
- Initiate and participate in case conferences/meetings with clients, family, service providers, and members of the interprofessional team.
- Act as client advocate to protect and promote clients' rights to autonomy, respect, privacy, dignity, and access to information.
- Registered Nurse/Registered Psychiatric Nurse with a valid practicing license in British Columbia.
- Experience working with individuals with mental illness and concurrent disorders.
- Strong communication and interpersonal skills.
- Ability to work collaboratively within an interdisciplinary team-based model of care.
