We are seeking a highly skilled and motivated Pharmacist to join our team at Fraser Health Authority. As a Pharmacist, you will play a critical role in providing safe and effective medication management services to our patients.
Key Responsibilities- Medication Management: Interpret medication orders, assess prescriptions for therapeutic suitability, and ensure that medications are efficiently and accurately dispensed and/or compounded.
- Communication: Communicate with physicians, patients, and/or other healthcare team members for prescription clarification and problem resolution.
- Medication Counselling: Provide instructions to others on proper use/administration, storage and/or disposal, possible side effects of prescribed and self-selected medications.
- Quality Improvement: Participate in the collection of data for medication use evaluation projects, implementation of dispensary-based medication safety initiatives, and quality control activities.
- Education and Experience: Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
- Skills and Abilities: Broad knowledge of organization policies, procedures, and standards of care; knowledge of evidence-based clinical pharmacy practice; ability to apply clinical pharmacy knowledge and make decisions.
