JOB PURPOSE

The Nurse Practitioner (NP) is responsible for providing comprehensive primary health care services, chronic disease management, and community development within an interdisciplinary team. The focus is on supporting communities in meeting their health needs through a population health approach.

Operating in various settings such as Community Health Centres, Health Centres, and collaborative practices, the NP delivers care to scheduled and unscheduled patients, those diverted from emergency departments, and vulnerable populations.

  • Direct care provided to individuals through different health facilities.
  • Emergency/crisis intervention and stabilization services offered.
  • Establishes relationships with key stakeholders in the government and community.

Accountable for meeting NP guidelines and professional standards.

QUALIFICATIONS

  • Bachelor's degree in nursing and completion of a Nurse Practitioner program at the Masters level.
  • 5 years of nursing experience with recent NP experience.
  • Registered with NANB as a Nurse Practitioner.
  • Current BCLS certification.
  • Extensive knowledge in client care across the health continuum.
  • Strong communication and organizational skills.
  • Ability to work effectively in a team and independently.
  • Skilled in decision-making and problem-solving.
  • Ability to apply theory and research to practice.
  • Experience in integrating health determinants and harm reduction approaches.
  • Committed to a non-judgmental, recovery-based approach.
  • Good attendance and work record.
  • Flexible to travel within the region as needed.
  • Regular attendance at work required.

KEY RESPONSIBILITIES

Health Assessment and Diagnosis:

  • Conducts comprehensive health assessments.
  • Orders and interprets diagnostic tests.
  • Establishes diagnoses and initiates treatment plans.
  • Manages acute and chronic conditions.
  • Collaborates with primary care physicians.
  • Refers patients for specialized care when needed.

Health Promotion and Prevention of Illness and Injury:

  • Provides health promotion services.
  • Implements prevention strategies for patients.
  • Participates in community health initiatives.

Professional Responsibility and Accountability:

  • Maintains competencies as per NANB standards.
  • Accountable against benchmarks and guidelines.
  • Participates in interdisciplinary care.
  • Promotes excellence in nursing care.
  • Utilizes research for evidence-based practice.
  • Establishes referral networks.
  • Acts as a mentor to peers and students.
  • Commits to professional development.
