Company Description
North Okanagan Hospice Society in Vernon, BC is dedicated to supporting individuals with a palliative diagnosis, their families, and the bereaved. Operating Hospice House, a 12-bed palliative care facility, we provide short-term respite care and grief/bereavement services. With trained volunteers, we offer comprehensive hospice palliative care services, engaging and supporting the community through education and programs.
Role Description
The Care Team Lead is responsible for responsible for the coordination of day-to-day operations and quality individual care within North Okanagan Hospice Society’s clinical area(s) and Hospice at Home. This position supports the seamless movement of individuals through the North Okanagan in accessing appropriate resources that promote excellence in the delivery of interdisciplinary hospice palliative care. The Care Team Lead supervises as well as serves as a role model and a resource to staff for clinical decision-making, clinical assessments of individuals and in the counseling and education of families. Liaises with the interdisciplinary team for identification, coordination, and efficient utilization of resources as well as plans for staff educational needs.
Duties and Responsibilities
· Responsible for the day-to-day operations, supervision of unionized staff and practicum students and quality client care.
· Identifies and ensures the completion of mandatory educational needs of staff. Collaborates with educational personnel regarding identified needs and plans the opportunities for professional development. Delivers training in collaboration with the Hospice Manager and Education and Resource Leader.
· As an integral member of the site management team, participates in decision-making - particularly in matters related to health and safety, resident care, financial, and human resources management.
· Ensures that there are effective communication systems in place for staff, clients and families.
· Supports the Hospice Manager to ensure proper maintenance of the clinical components of the residential facility, medical equipment, and furnishings.
· Reviews referrals and ensures timely admissions and discharges in coordination with care team, Interior Health partners, and other relevant multidisciplinary team members.
· Perform all other duties as required.
Leadership:
· Reviewing, analyzing, and making recommendations on the development of administrative systems such as policies, procedures and standards of patient care and adequate staffing levels.
· Acts as a change agent within the service, fostering an environment of innovation and critical thinking.
· Supports the Hospice Manager to ensure effectiveness of care provided and seeks mechanisms to ensure that standards are met consistently.
· Developing and maintaining a continuous quality improvement environment.
· Participates in the current trends and data in Palliative End of life care related to the provision of accessible, comprehensive, efficient, and effective service/care.
· Participates in coordinating the utilization of shared resources including services, equipment, and space with other departments and/or sites.
· Collaborates with interdisciplinary team members internally and externally in planning and provision of effective and efficient care within palliative end of life portfolio to support the achievement of operational and strategic objectives.
· Maintains a positive and productive work environment respecting the value of an interdisciplinary team approach to the provision of health services.
· Shares information and participates on committees to develop and implement best practices throughout the organization. Represents the organization on a variety of internal and external committees as required.
· Participates in Leadership Team weekly On-Call roster and schedule. Covers for Hospice Manager during leaves.
Human Resources:
· Supports the recruitment, hiring and orientation of staff.
· Support the Hospice Manager in the administration of the collective agreement and may represent the employer in the grievance process, on various union/management committees, and during essential service situations where needed.
· Supports the Hospice manager with coordination of the team in staffing, scheduling, and vacation planning.
· Provides mentoring and coaching to staff, and delivers staff development, training, and succession plans.
Safety and Compliance:
· Ensures workplace safety.
· Lead the JOSH committee and adhere to safety and Work Safe BC regulations and standards.
· Supports the Hospice Manager to ensure Licensing and Legislation requirements are met for buildings, and clinical equipment is maintained to licensing standards.
Budget and Financial:
· Supports the Hospice Manager with input into budget, identifies variances, and takes corrective action as required to maintain expenditures within the approved budget(s).
Qualifications
- Team Leadership and Training skills
- Clinical Operations expertise
- Strong interpersonal and communication skills
- Ability to work effectively in a multidisciplinary team
- Bachelor’s degree in nursing and five years’ nursing experience in palliative, long term and / or community care within the last seven years or equivalent combination of education, training, and experience.
- Two years recent supervisory and teaching experience or an equivalent combination of education, training, and experience.
- Graduation from an approved School of Nursing with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).
- Computer literacy in a Microsoft Windows environment including Microsoft 365 suite.
- Knowledge of palliative care practices; Victoria Hospice training and/or CHPCA certification an asset
