Summary:

Reporting to the designated Occupational Therapy Site Leader, and receiving guidance and support from the Professional Practice Leader and/or Research, Education and Practice Coordinator this position:

  • Provide advanced clinical advice for at-risk geriatric populations
  • Develop, plan, and implement treatment and care plans
  • Communicate treatment plans to the Seniors Outreach and Consult Team
  • Coach and mentor staff on the needs of geriatric patients
  • Collaborate with community partners for successful patient transitions
  • Develop education programs and resources for healthcare staff, patients, and families
  • Lead and evaluate clinical activities and monitor patient outcomes
Qualifications / Skills and Education:

Education:

  • Baccalaureate Degree in Occupational Therapy.
  • Four (4) years’ recent related experience in an acute and/or outpatient clinical setting, including two (2) years’ experience working with the specialized needs of geriatric patients, plus completion of additional post-graduate courses related to gerontology and treatment of the elderly, or an equivalent combination of education, training and experience.
  • Current full registration as an Occupational Therapist with the College of Health and Care Professionals of BC (CHCPBC). Eligible for membership with the Canadian Association of Occupational Therapists.

Skills and Abilities:

  • Comprehensive knowledge of Occupational Therapy theory and practice within a client and family centered model of care.
  • Comprehensive knowledge of clinical practice related to treatment and care of geriatric patients.
  • Demonstrated ability to work collaboratively as a member of an interdisciplinary team.

  • Ability to develop and build strong working relationships.

Your Day to Day:
  • Develop, plan, and implement advanced occupational therapy assessments for at-risk geriatric patients.
  • Collaborate with interdisciplinary teams, patients, and families to create and manage ongoing care plans.
  • Work with community partners to ensure necessary supports for patients returning home, including arranging supplies and equipment.
  • Develop educational materials for patients, families, and healthcare team members.
  • Use various teaching strategies to support education activities and conduct presentations, demonstrations, and in-service sessions.
  • Collaborate with inter-professional teams to develop and plan programs and services for geriatric patients.
  • Participate in and lead continuous quality improvement activities, evaluating clinical services and patient care practices.
  • Establish positive working relationships with stakeholders, including community partners and hospital staff.
  • Coach and mentor patients, families, and staff, providing feedback to enhance patient care.
  • Maintain and update clinical and professional knowledge of geriatric occupational therapy practices and participate in relevant education programs.
