We are looking for a Licensed Practical Nurse with experience in patient-centered, trauma-informed, and recovery-based care for the Addiction and Mental Health Outreach Team in Lethbridge, Alberta. This position is based out of the Lethbridge Provincial Building and will provide concurrent support to individuals who require treatment in place for a variety of medical conditions, including chronic disease, acute wound care and STBBI testing/treatment. Services will be community-based and recovery-oriented toward adults experiencing severe and persistent mental illness, addiction, and homelessness. You will work in collaboration with community partner teams, both Recovery Alberta and non-Recovery Alberta based, to collaboratively provide care to individuals whose lives have been impacted by unstable housing, socioeconomic factors, and substances. Working within the Licensed Practical Nurse scope of practice, you will provide consultative assessments and intakes for clients to determine physical and mental health stability, complete thorough health assessments, treatment planning, management, and monitoring of both patient physical and mental health issues. You will demonstrate a knowledgeable and caring clinical practice that is grounded in addiction and mental health nursing from a recovery-oriented perspective; have strong communication and interpersonal skills; demonstrate time management, independent decision-making, critical thinking, and problem-solving skills; and an ability to foster positive relationships within a team environment are all expectations of this role.

As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Demonstrated ability to work independently. Experience working with vulnerable populations. Experience working with clients with Addiction & Mental Health 3 years. Experience providing outreach care for 3 years.

2+ years working with homeless populations.
