UHN’s Transitional Care Program (TCP) includes UHN’s Buttonwood Reactivation Care Centre (RCC), Hillcrest Reactivation Centre & Queens Estate Transitional Care units. The RCC aims to improve restoration to help patients maintain social, cognitive and physical functioning by leveraging patient-specific, goal-based service planning. Patients serviced by the TCP do not require acute care but need to engage in activities to improve their function and independence, provided less active medical management and more therapy and activity. TCP also serves several health system purposes, including:

The (APNE) works within the Collaborative Academic Practice & Practice-Based Education portfolios.

Reporting to the Manager of Professional Practice, the APNE aligns with unit interdisciplinary leadership teams and demonstrates a set of core competencies fundamental to developing and maintaining the academic practice environment at UHN. The competencies focus on education and staff development, interprofessional collaboration, scholarly/research activity, and leadership/change management.
