Detailed Overview

The Associate Clinical Pharmacy Lead (ACPL) works collaboratively with the Coordinator, Clinical Pharmacy Services to advance clinical pharmacy practice and realize the vision of direct patient care in accordance with the philosophy, goals, and objectives of Lower Mainland Pharmacy Services.

The ACPL assists the Coordinator, Clinical Pharmacy Services in promoting a progressive clinical pharmacy service in developing, leading, implementing and evaluating clinical pharmacy services within the site including pharmaceutical care, drug therapy monitoring, staff orientation and continuing pharmacy education.

In addition, the ACPL provides pharmaceutical care and maintains a clinical practice in an assigned clinical area.


Responsibilities

  1. Collaborates with Coordinator, Clinical Pharmacy Services to provide input and recommendations on new or revised clinical pharmacy services, policies and procedures for the designated area.
  2. Assists the Coordinator, Clinical Pharmacy Services in the daily operations and activities of the clinical pharmacy service including establishing work schedules and priorities for the pharmacists providing clinical pharmacy services.
  3. Participates in developing orientation programs for new staff and provides orientation to clinical pharmacy services for the designated area; serves as a role model to other staff in the demonstration of performance objectives, work ethic and adherence to department policies.
  4. Acts as a preceptor for students, residents, doctorate candidates, undergraduate and graduate students.
  5. Monitors quality assurance and performs workload measurement of tasks in the assigned areas to ensure consistency within the department.
  6. Participates as an interdisciplinary team member by communicating with Pharmacy team members, physicians, nurses and other professionals; provides information regarding products and operational policies, investigates and responds to complaints and attends meetings in order to ensure coordinated efforts of all functions pertaining to the designated area.
  7. In conjunction with the Coordinator, Clinical Pharmacy Services, recommends budget adjustments, as appropriate.
  8. Provides pharmaceutical care to patients by attending rounds, performing medication assessments, monitoring drug therapy, documenting in the patient record, assessing laboratory values and participating in drug use evaluation projects.
  9. Performs other related duties as assigned.

Qualifications

Education, Training and Experience


Current registration as a Pharmacist with the College of Pharmacists of British Columbia.

Certificate of completion of an accredited Hospital Pharmacy Residency Program and a post graduate Doctor of Pharmacy Degree from an accredited university, supplemented with three (3) years' recent, relevant hospital pharmacy experience, or an equivalent combination of education, training and experience.

Skills and Abilities

  • Knowledge of evidence-based clinical pharmacy practice.
  • Knowledge of medication use evaluation.
  • Knowledge of research processes and methodology.
  • Ability to independently recommend, implement, monitor and alter pharmacotherapeutic plans based on patient-specific information.
  • Ability to teach within a multidisciplinary setting.
  • Ability to use focused physical assessment to evaluate efficacy and/or toxicity of drugs.
  • Ability to retrieve and evaluate drug literature to provide patient-specific drug information to health professionals and patients, in support of critical care pathways and drug use evaluations.
  • Ability to effectively lead and coordinate project teams, pharmacists and students.
  • Ability to communicate effectively both verbally and in writing, including the ability to make presentations.
  • Ability to establish and maintain effective working relationships with other health practitioners, as well as patients, family members and the public.
  • Ability to plan, implement, organize, problem-solve, and manage projects.
  • Ability to organize work and prioritize work.
  • Ability to operate related equipment.
  • Physical ability to carry out the duties of the position.
