The Wiigwas Elder and Senior Care (WESC) Attending Nurse Practitioner (NP) is a primary care provider to WESC residents who works to their full scope of practice as described and outlined by the College of Nurses of Ontario (CNO). The attending NP will practice both independently and in collaboration with other members of the health care team, residents and resident families/caregivers in the development, implementation and evaluation of resident care plans throughout various illness trajectories including: health promotion, injury and illness prevention, episode and acute care, chronic disease/illness management, restorative/rehabilitative care and palliative care. The Attending NP is an on-site provider, using 70% of their time to provide direct resident care and the remaining 30% of their time for education, quality improvement, research, and leadership activities. The attending NP will contribute to:
• Continuity of Care
• Creation and implementation of Quality Improvement Plans
• Research and implementation of evidence-based practices
• Increased knowledge capacity of staff and programming to meet the needs of the residents
• Improved care coordination and collaboration across the care continuum
• Internal and external committees on behalf of Wiigwas Elder and Senior Care
The Attending Nurse Practitioner (Attending NP) reports directly to the Director of Care or Administrator and is also accountable to the Medical Director for meeting the long-term care home's (LTCH) policies, procedures, and protocols for medical services.
Ø Perform comprehensive health assessments Ø Order and interpret diagnostic tests (including CT, MRI, Ultrasound, x-ray, laboratory investigations, etc), to diagnose health problemsØ Clearly communicate a diagnosis and develop a treatment plan based on diagnostic interpretation and clinical findingsØ Provide treatment and ongoing monitoring of acute and chronic health conditions that include comprehensive pharmacologic and non-pharmacologic therapiesØ Provide referrals to other care providers, specialists and specialized clinics when necessaryØ Collaborate with all members of both Wiigwas's primary care health team, service providers (physicians, nursing, dietitians, physiotherapy, pharmacy, emotional wellness) from other organizations Ø Conduct screening for early detection of cancers and chronic diseases (i.e., pap testing, FIT testing, mammography, diabetes screening) and provide preventative health teachingØ Maintain and protect resident confidentiality at all timeØ Document resident care within the electronic medical record in a timely manner as outlined by the College of Nurses of Ontario practice standardsØ Respond to critical laboratory results or diagnostic findings ordered and provide timely follow-up to clients.Ø Provide cross-coverage to NP/MD colleagues within the health care team; colleagues will also offer cross-coverage so that appropriate vacation and time off without interruption is allotted
Accountabilities:
Ø Maintains confidentiality and acts in an ethical manner consistent with professional expectations and the LTC's policies and code of conduct at all times Ø Acts as a collaborative member of the primary care team, seeking appropriate consultation/referrals to specialists and/or allied health care professionals when requiredØ Engages in ongoing professional development in order to maintain clinical competence and currency with legislation affecting their practice (i.e. CNO practice standards, Fixing LTC Act) Ø Develops in-depth expertise in the care of older adults, dementia care and palliative care Ø Develops and maintains expertise in leadership Ø Holds professional libality protection Ø Maintains CPR certification for health care providers Ø Maintains registration with the College of Nurses of Ontario Ø Maintains membership with applicable professional associations (NPAO, RNAO)
Roles and Responsibilities:
Ø Perform comprehensive health assessments of LTC residents for a broad range of health conditions and across illness trajectoriesØ Ability to autonomously order and interpret diagnostic tests (including CT, MRI, Ultrasound, x-ray, laboratory investigations, etc) within the legislated NP scope, to diagnose health problems Ø Provide treatment and ongoing monitoring of acute and chronic health conditions that include comprehensive pharmacologic and non-pharmacologic therapies based on resident/caregiver goals of care Ø Implement strategies to optimize resident health and quality of life in the areas of illness and injury prevention, health promotion, health maintenance, rehabilitation and restorative care and palliative care Ø Provide referrals to allied health care providers, specialists and specialized clinics when necessaryØ Possess advanced knowledge, skill-set and expertise in the resident-focused care and the management of older adults with medical complexity, frailty and cognitive impairment Ø Initiate and participate in ongoing discussions with residents and families/caregivers regarding advanced care planning, goals of care and end-of-life care planning Ø Collaborate and maintain clear communication with all members of the resident care team and caregivers/families Ø Document client care within the electronic medical record (point click care) within a timely manner as outlined by the College of Nurses of Ontario practice standards Ø Respond to critical laboratory results or diagnostic findings ordered and providing timely follow up to clients Ø Provide ongoing mentorship, knowledge-translation, and education to registered and non-registered staff in line with evidence-based guidelines to support clinical best practice Ø Consistently engage in evidence-informed practice and identify and implement research-based innovations to improve resident care Ø Regularly attend and participate in resident care conferences Ø Regularly attend and participate in professional advisory and quality committee activities (i.e. palliative committee, falls committee, skin and wound committee) as well as weekly care rounds/huddles Ø Participate in regular development, implementation, and evaluation of facility policies and proceduresØ Demonstrate knowledge, understanding and utilization of the Fixing Long Term Care Act, 2021Ø Possess a broad understanding of the health care system and have the ability to collaborate with other health care organizations within the Northwest LHIN region
