DUTIES AND RESPONSIBILITIES

General duties and responsibilities included are not exclusive or limited to the following.

Respond to client-related requests (such as registering for services, discussing service options, requesting information, and providing rapid response counselling).
Oversee the client's case from beginning to end, including coordination of appropriate support, information management related to the case, resolving client concerns, and facilitating relationships between the client and appropriate service providers/partners.
Function as a liaison between clients, their treatment team, and other health agencies. Partner with health care professionals to deliver exceptional client service.
Perform client Triage/intakes and assessments to determine needs.
Conduct brief telephonic counselling support, as appropriate.
Facilitate warm transfers of callers in distress to crisis services or to on-call counsellors.
Systems navigation, care coordination, communication, and collaboration with the client's circle
Build relationships and trust with the clients to ensure that clients are receiving proper care and treatment in the services they require and must maintain confidentiality at times.
Provide tailored support for clients throughout their lifecycle, including systems navigation, care coordination, communication, and collaboration with the client's circle of care to ensure best outcomes for client wellness.
Provides counselling services to individuals calling into NAN Hope during times of crisis.
Develops new programming initiatives to improve the overall wellness of clients serviced.
Provides group programs and services to clients such as AA or Elders Care Groups.
Participates in community development and delivers community education sessions.
Maintains accurate and confidential client records electronically.
Carry out thorough risk assessments and monitoring of clients.
Collaborate with other professionals to develop treatment plans for clients.
Assist clients to manage their emotions through de-escalation strategies.
Use trauma-informed best practices.
Provide regular updates to the Director of eHealth and the Clinical Services Coordinator.
Build relationships with community health staff and community leadership.
Crisis response, intervention, and promote harm reduction.
Provide pandemic support.
Attend mandatory team meetings.
Travel as needed for community engagement sessions to NAN communities.
Receives initial and ongoing training as needed and as available.
Provides training as necessary for community staff.
Travel to other KO sites such as Thunder Bay/Balmertown as needed.
Perform all other duties as assigned.
COMPETENCIES AND QUALIFICATIONS
Master of Social Work Degree or equivalent is preferred but degree with registration is required.
Professional experience in Psychology, Social Work/Social Services, Health, Healthcare Administration an asset.
Crisis/distress/de-escalation training and experience.
Excellent verbal, written and interpersonal communication skills.
Exceptional client service skills
Ability to effectively and efficiently problem solve and multi-task.
Knowledge and understanding of Indigenous traditions, values, and socioeconomic barriers that impact First Nation Peoples.
Ability to speak Cree, Oji-Cree or Ojibway and experience working with First Nations communities an asset.
Familiarity with the health system and health care providers in the district. Travel to remote communities required.
Strong oral, written and interpersonal communications skills, including report writing and ensuring proper documentation.
Proficiency in computer programs such as M/S Word, Excel, PowerPoint, and the use of email; ability to learn communications technology and systems software quickly.
Ability to work independently with minimal supervision or as part of a team.
Ability to adapt effectively to changes in workload.
Adheres to strict company privacy and confidentiality policies and follows the Personal Health Information Protection Act (PIHIPA).
Valid Class G Driver's License and must be willing to provide a police criminal background check completed within the last 6 months.
