Responsibilities:
Provide clinical services to patients, including medication therapy management, immunizations, and health screenings.
Conduct comprehensive medication reviews to optimize therapy and minimize adverse effects.
Collaborate with healthcare providers to develop and implement care plans for patients with complex medication regimens.
Evaluate the appropriateness, safety, and efficacy of prescriptions based on patient needs, collaborating with healthcare providers as necessary.
Ensure prescriptions are legally and accurately processed, providing counselling and referrals as needed.
Maintain communication with healthcare professionals, insurers, and customers to clarify information and address concerns promptly.
Document patient interactions and consultations to comply with pharmacy legislation.
Initiate, modify, and monitor drug therapy for chronic diseases, educating patients and caregivers on management strategies.
Advocate for appropriate diagnostic tests and encourage self-monitoring tools for disease management.
Assist patients in selecting appropriate self-care regimens, considering their medication regime and health condition.
Education and Experience:
Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
A member in good standing with the Alberta College of Pharmacy, including a valid Alberta Pharmacist license
Completed an internship or fellowship within a retail pharmacy environment
Additional Prescribing Authority or willingness to attain
Valid injection Certification or willingness to attain
Qualifications and Skills:
Strong clinical knowledge and understanding of pharmaceutical principles.
Excellent communication and interpersonal skills.
Ability to work collaboratively in a multidisciplinary healthcare team.
Commitment to ongoing professional development and continuing education.
Demonstrated empathy, cultural sensitivity, and a passion for patient-centered care.
Remains current with new advances in health and pharmacy-related matters and pharmaceutical products.
Very strong attention to detail
Computer proficiency with Word, Excel spreadsheet, Operis, NeTcaRe, Kroll and other database programs.
