TITLE:
Intake Specialist/Passport Team Lead
QUALIFICATIONS:
Degree or diploma in Developmental Services or related field; excellent communication, organization, and time management skills; Computer Skills using Microsoft Office Software (Excel spreadsheets); driver's license and required insurance; First Aid & CPR; knowledge of persons with disabilities and individualized planning, experience in supporting and working with families, ability to facilitate personal plans, knowledge of community resources, creative and solution focused approaches, comfortable with public speaking.
JOB SUMMARY:
In keeping with the philosophy of inclusion and the mission of Hutton House, the Intake Specialist/Passport Team Lead will support participants and their families/caregivers through managing Passport Funding and scheduling and conducting intake meetings.
This role involves working evening and weekend hours as needed in order to best meet the needs of participants and their families/caregivers.
Passport Team Lead includes leadership of Passport staff and scheduling work hours.Reporting Relationship:

Intake
- Schedule and implement intake meetings either one on one, with a family/caregiver, or a referring agency.
- Complete all required forms for Hutton House General Intake and community referrals.
- Answer general questions about Hutton House programs, assist with initial registration process for both internal and external programs.
- Inform Hutton House Staff of participant needs after intake by attending team meetings, meeting with staff individually, phone contact etc.
- Complete and follow up on all required documentation ie reference checks, Release of Information contacts, Consents for Disclosure of Personal Health Information, etc.
- Gather information on a regular basis regarding program availability, waiting lists etc., in order to provide current information.
- Using agency software, ensure all required documentation is completed on a timely basis.
- Ensure all necessary Intake Forms are developed to meet program needs.
- Follow FOCUS Accreditation requirements.
Passport Team Lead
- Facilitate a person directed planning process with individuals and/or their families/caregivers, listening to and respecting their wants, needs and interests.
- Monitor and evaluate goals and complete an annual report to ensure continuous improvement of Passport services and positive outcomes for each individual.
- Utilize a variety of internal and/or external resources/information such as Hutton House or community opportunities to support each individual to achieve their personal goals.
- Provide information to Passport participants regarding administrative supports available to manage funds ie monitoring expenses, bill payments, support worker payroll, issuing T4s etc;
- Coordinate support staff schedules, payroll, training and annual performance reviews.
- Mentor and coach Support Workers to ensure participant goals are met and to develop new ideas as participants set new goals.
- Provide ongoing assistance to ensure Passport funds managed through Hutton House are utilized before the end of fiscal year.
- Track payments, invoices, reimbursements and purchases for all Passport participant using Excel spreadsheets. Ensure monthly invoices are submitted to the PassportONE portal.
- Collaborate with the HR Generalist and Manager of Community Services on the recruitment process for hiring a supporting worker.
- Liaise with Community Services Coordination Network (CSCN) and Developmental Services Ontario (DSO) as needed.
- Work with the Manager of Community Services to ensure Passport program is in line with the Journey to Belonging.
Community Outreach
- Keep informed of external community services and supports in order to make appropriate referrals and provide good customer service.
- Source new opportunities to inform the community about Hutton House programs and services and attend community fairs, create presentations etc. on behalf of Hutton House.
- Promotes the abilities of persons with disabilities and the services of Hutton House throughout the community.
- Maintain relationship with Passport Networking group.
Working Relationships
- Maintain a positive, supportive relationship with all staff, participants and volunteers.
- Advocate to ensure that the dignity and rights of persons with disabilities are advanced.
- Adhere to all policies of the organization to ensure a safe and supportive environment for participant.
- Maintain relationship with community partners.
- Contribute as a team member to the overall mission of the organization. Contribute to and support FOCUS Accreditation and the Strategic Plan as needed.
- Respect and adhere to confidentiality in all communications with others.
