POSITION TITLE: Supportive Living Coordinator

PROGRAM: Operations – Supportive Living (Group Homes)

POSITION TYPE: Permanent, Part-time

HOURS OF WORK: 28.5 hours per week (Thursday-Saturday 1 pm -10:30 pm)

POSITION SUMMARY: The Coordinator plays a vital role as the initial point of contact, offering guidance and assistance as individuals start their wellness journey. This position upholds high standards of care, delivering positive, empathetic service and problem-solving skills to support clients' needs.

REPORTS TO: Team Lead, Supportive Living

KEY SERVICE RESPONSIBILITIES AND SPECIFIC ACCOUNTABILITIES

  • The creation and assessment, in collaboration with the client and their support team, of a Service Plan with measurable objectives aimed at improving/sustaining independent living abilities.
  • To engage with community partners regularly to ensure joint efforts in aiding clients to achieve their personal objectives.
  • To help clients form peer, family, and community support connections.
  • To offer clients guidance and supportive counseling during urgent or crisis situations.
  • To stand up for clients when needed and ensure they receive proper care, treatment services, and information from the community and CMHA.
  • To promote a harmonious living environment through scheduled community/house meetings and individual interventions if required.
  • To uphold efficient written and verbal communication systems crucial for client service provision and program operation.
  • Performing other necessary tasks.

QUALIFICATIONS AND SKILLS

  • Minimum qualification – Bachelor of Social Work (BSW) or Bachelor in Psychology or equivalent diploma/degree in a human services field with relevant experience also considered.
  • Minimum of two (2) years of hands-on experience working with individuals with diagnosed mental health conditions, preferably in a skill-building or residential setting.
  • Prior experience working with individuals dealing with substance misuse is advantageous.
  • You have been active in the mental health sector for over two years and possess a strong dedication to and comprehension of the recovery approach.
  • Current certification in Applied Suicide Intervention Skills (ASIST).
  • Up-to-date First Aid and CPR certifications.
  • Must be available for varied work shifts.
  • Possession of a valid driver's license and access to a vehicle.
