We are seeking:

Care Team Scheduling Coordinator (Integrated Client and Community Care)

Permanent Full-time (35 hours per week)

1:30pm-9:30pm Shift - Monday-Friday with rotating weekends

Must be available to work rotating weekends.

What is the job?


As a Care Team Scheduling Coordinator you will help our clients maintain their independence and brighten their day by fielding client calls and ensuring their services are scheduled in a timely manner.

You'll work in a fast-paced high energy customer service centre, communicating with clients, families, funders and clients' Health Care Teams including Personal Support Workers, Nurses and Rehab Service Providers.

You'll be supported by a passionate team and state
- of-the art technology that enables you to coordinate services, accept new client referrals, schedule service changes and maintain client service plans.

Am I qualified?
Excellent communication skills that demonstrate empathy, compassion and a client and a family-centred approach are essential to this role. Solid time management skills, attention to detail and the ability to prioritize tasks are equally important. You must be able to work independently while being flexible enough to support the team as changes arise. Medical terminology would be an asset.

Satisfactory completion of the technical skills testing is required.
