Position Overview:

The Senior Support Worker ensures clients' well-being in their homes to promote seniors aging at home (in place).

This role assists clients in completing household chores (laundry, light cleaning, meal preparation), accessing and navigating the community, connecting seniors to eligible resources offered by other organizations in the community, providing travel support to medical appointments and other community locations, and assistance in understanding incoming mail and potential next steps. Bridging technological barriers to services and day-to-day living.

Organizes senior education opportunities with other agencies to increase senior education on a variety of topics from modified exercises, nutrition, fall prevention, accessibility resources (walkers) and other focuses that target an aging population.

Completes initial assessments of needs and quality of life and continues to evaluate and update assessments to determine further supports and effectiveness of supports in place.

Engages with volunteers to provide mentorship and assistance where needed.

Typical Functions and Responsibilities

1. Facilitates assessments of needs and quality of life with clients, their families, and healthcare providers to determine programs that meet clients' physical, home-management, recreational, social, and educational needs. Supports clients in their own homes, in programs, and in the community to allow them to function safely and independently, supporting them to age in place.

2. Provides supportive counseling using PSR principles, motivational interviewing, demonstrations, modeling, and behavior management to clients, their families, and/or personal networks to assist with the development of physical, social, emotional, and life skills such as maintaining personal hygiene, housekeeping, meal planning and preparation, meeting financial obligations, making/keeping appointments, and interpersonal skills. Accompanies clients to medical appointments to provide support and consistency when navigating day-to-day management of illness.

3. Monitors clients and recognizes potential crisis or emergency situations, analyzes, and develops strategies to deal with such situations to ensure no harm to the client, staff, and/or the public. Reports medical, behavioral, and other problems, as required.

4. Identifies available social, economic, recreational, and educational services in the community that will meet clients' needs; participates in group planning and helps develop individual activity plans which focus on the special needs of each client. Provides clients with related information.

5. Maintains written records and documentation regarding client activities and progress, for example: care plans, progress reports, statistics, and daily logs; ensures all required documentation and reports are up to date and accurate.

6. Facilities clients' physical, recreational, and home-maintenance needs by performing duties such as arranging for clients' transportation to programs/services, assisting clients to attend appointments, and implementing programs that meet the client's needs.

7. Maintains liaison with families, other agencies, professionals, government officials, and the community, and monitors progress and well-being of clients.

8. Monitors clients and provides medical reminders; discusses' the importance of medication and how it contributes to wellness along with other strategies. Reports to a clinical body when concerned about a client's medication regime status.

9. Advocates for clients by problem-solving in areas such as property owner disputes, financial assistance, assessing community resources, and barriers to aging in place.

10. Ensures that all requirements are maintained to provide safe transportation, proper driver's license, car maintenance, etc. Maintains up-to-date knowledge of building fire regulations and procedures and other emergency procedures; ensures all health and safety standards are maintained.

11. Performs other duties, as assigned.

12. Identify and assist in the implementation of fall prevention strategies both environmentally and unique to individual physiological needs. Accessing community resources as required

13. Identifying and addressing the complex issue of abuse and neglect of seniors and implementing a range of activities such as applying laws and raising awareness of rights within these laws.

14. Educating clients and lesioning with community resources related to estate planning and medical care levels related to complex medical issues.

Other Certificates, Designations or Licenses:

1. Vulnerable population's criminal record clearance

2. Any other qualifications determined to be reasonable and relevant for the level of work

3. Have own vehicle preferred

Technical Skills:

1. Proficient with computer software applications e.g. MSOffice

6. Proven physical ability to perform duties of position

Other Preferred Experience

1. Experience working with individuals with a diagnosis of co-occurring disorders.

2. Additional training/experience in psychosocial rehabilitation.
