Reporting to the Program & Support Services Manager, the Social Worker is responsible for the development

and implementation of the Social Work Program.

The goal of the Social Work program is to ensure that residents are provided with a safe, person-centered, and strengths-based plan of care consistent with the Ministry of Health/Ministry of Long-Term Care standards, and compliant with professional accountabilities of the Ontario College of Social Workers and Social Service Workers.

What you'll do

  • Complete the following activities within six weeks o the admission process: MMSE or MoCA, Geriatric Depression Scale or Cornell, provide adjustment support, as needed, and contact resident's POA/SDM to offer adjustment support.
  • Increase continuity of care through collaboration, consultation, and referral as appropriate
  • Participate in resident care conferences and family meetings.
  • Advocate for and provide palliative and endoflife care.
  • Provide grief and bereavement support to residents and families.
  • Assist residents in obtaining capacity assessments and setting up POA/SDM and PGT, where applicable.
  • Document clinical data, assessment findings, plans of care, therapeutic interventions, resident responses, in a timely and accurate manner.
  • Engage residents in dialogue to determine what is important to them for health and quality of life.
  • Support the Family Council as the Liaison.
  • Coordinate the Music & Memory Program.
  • Participate in Committees as appropriate.
  • Provide formal and informal teaching and coaching to interdisciplinary team members, serving as resource person, and educator.
  • Participate in identifying, analyzing, and interpreting trends to determine priorities for educational programming.
  • Other duties as assigned

What you'll bring

  • Bachelor of Social Work Degree or a related field from a community college or university
  • Member of the Ontario College of Social Workers and Social Services Workers, in good standing.
  • Minimum two (2) years relevant experience in a health care setting, longterm care experience preferred.
  • Ability to work collaboratively as a member of a multidisciplinary team.
  • Ability to engage with a wide variety of people with a high level of professionalism, compassion and courtesy including culturally, socially, and economically diverse populations, seniors, persons with disabilities and professional colleagues.
  • Excellent organizational, time management and communication skills.
  • Demonstrated client centered approach to care.
  • Ability to ensure confidentiality and privacy of resident information.
  • Commitment to excellence in professional practice and continuous professional learning
  • Maintain good work attendance and is flexible with time.
