Scope


Responsible to deliver components of My Pathways to Employment, Women in Food and Customer Service, and Wrap Around Support (WAS) Programs.

This position will serve vulnerable Ontario Works (OW) clients in Toronto by carrying out needs assessment, develop personal plans, mentoring services, service navigation, referrals to health and social supports like basic needs and housing to ensure clients wellness and mental health.

Furthermore, the role will require facilitating workshops, delivering Core Program and Post Program components and individual employment-related counselling.

Responsibilities
The position's key responsibilities include, but not limited to:

  • Provide the full range of activities related to My Pathways to Employment, Women in Food and Customer Service, and Wrap Around Support (WAS) to vulnerable Ontario Works (OW) clients in Toronto referred by the Toronto Employment Essentials Services (TESS).
  • Conduct needs assessment to identify immediate needs and potential barriers for each family member so that service delivery can be tailored to meet client needs
  • Responsible for service planning to develop a personal plan and identify the internal and external referrals needed to achieve set goals
  • Support clients in the decisionmaking process through information provision and supportive counselling
  • Facilitate referrals and linkages to address the needs and barriers identified
  • Responsible for regular followups to check on progress, identify emerging needs, additional services needed, changes to the plan and further linkages and referrals
  • Organize and facilitate information dissemination sessions for individuals or group sessions
  • Maintain strong working relations with agency programs and partner agencies
  • Effectively track services in internal and external databases and provide accurate reporting for supervisor
  • Maintain continuous and accurate records of clients assisted and services provided, in electronic database and paper files
  • Effectively track clients' progress and provide reports for supervisor, colleagues
  • Facilitate program activities according to set model and funder requirements
  • Coordinate courses and workshop schedules and guest speakers
  • Create, implement and revamp curriculum to enhance programs
  • Conduct intake and orientation for new clients and identify a fit to the program
  • Ensure clients are successfully completing the Core Program component to utilize annual seats allocation
  • Develop a comprehensive action plan for each client in meeting their career goals
  • Plan and deliver Post Program activities for clients to progress in their plan of action to meet their employment related goals
  • Meet the outcome rate for program effectiveness set by the organization and a funder
  • Providing comprehensive information on labour market, regulated professions and trades and professional licensing to clients and support clients in their pathway to accreditation
  • Critiquing clients resumes and cover letters, conducting mock interviews
  • Plan and conduct outreach to ensure individuals and organizations are aware of Polycultural' s services
  • Negotiate and monitor work/placement opportunities with employers and industry groups
  • Oversee contractual agreements between clients and employers and process financial incentives
  • Maintain strong relationships with OW case workers and update them on clients' progress
  • Attend networking and outreaching events to promote program awareness
  • Must meet targets set by funders and program manager
  • Other duties as assigned
