Detailed Overview:

Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families:

The Consultant, Clinical Standardization & Integrated Plan of Care (IPoC) promotes clinical best practice and quality patient outcomes by providing leadership and guidance to clinical directors, managers, clinicians, and health care teams regarding clinical standardization, point of care documentation and the IPoC initiative.

Provides expert advice and problem solving for clinical practice areas participating in the IPoC initiative; in collaboration with stakeholders, identifies barriers to implementation, develops appropriate action plans and evaluates outcomes.


Responsibilities:

  • Provides consultation leadership and guidance to clinicians, managers and directors regarding clinical standardization, care standard and principles adoption and the development, implementation and evaluation of the Integrated Plan of Care (IPoC) Initiative.
  • Works to support compliance with clinical practice established through care standard and IPoC implementation plans. Ensures proper governance is established and processes are followed. Develops an education plan, clinical decision support tools, revision of supporting documentation, and a plan to support the various operational teams and physicians impacted by changes to practice.
  • Oversees and supports related projects and initiatives, incorporating change management and knowledge translation considerations.
  • Oversees clinical standardization and IPoC staff as designated and related projects as assigned including the supervision of day to day operational work.
  • Maximizes health human resources utilization by facilitating full scope of practice for all categories of health providers.
  • Recommends and leads or contributes to the development of new clinical policies and procedures.
  • Obtains, reviews and disseminates current information including IPoC governance committee processes, IPoC directives and targets, project plans, education requirements.
  • Represents Fraser Health and the IPoC initiative at various events including educational forums, committees.
  • Networks, understands and supports all IPoC structures and related clinical groups such as Core Team members and clinical teams, for the primary purpose of practice integration and facilitating consistency in standards of care and point of care documentation.

Qualifications
:
Education and Experience

Graduate of an approved clinical health profession with a Bachelor's degree in health administration, education, or related health field.

Seven (7) years' recent, related clinical and/or education experience in a complex health care environment including project management and leadership experience, or an equivalent combination of education, training and experience.

Current practicing registration with the relevant college or association in British Columbia.
