The Opportunity:
The Point of Care (POC) Mentor is a leader within the interprofessional care team that consistently demonstrates excellence in practice in all domains of the Youth Care Worker (YCW) position.
The POC Mentor will support the ongoing development of the YCW practice across the MHA program and in the community.
The POC mentor will work with YCWs to promote skill development and consistency in practice, as an interprofessional team member who is providing care and treatment to children, youth and families.
The POC mentor will identify practice development needs of YCW I and YCW II skills and abilities in their youth care worker role.
The POC mentor provides direction and guidance to YCWs to ensure the endorsed best practice model approach and skills are evident in practice while supporting children, youth and families.
The POC mentor acts as a YCW role model and coach at the point of care and is accountable to regulated healthcare professionals.
Key responsibilities include, but are not limited to:
- Under the direction of the interprofessional care team of regulated professionals, the POC mentor role models, supports and provides feedback around the YCW practice with the goal of enhancing skill and performance
- Identification of YCW I and YCW II opportunities for practice improvement.
- Supports YCW development of practice in milieubased and communitybased settings
- Provides in the moment feedback on treatment plan implementation, client communication and the transfer of knowledge to practice.
- Observes and assesses YCW practice to ensure YCW interactions with clients and families are supportive, trauma informed and therapeutic in nature.
- Role Models and coaches YCWs to understand the meaning behind the behaviour and the most effective approach by integrating expertise in Nonviolent Crisis Intervention (NCI), Collaborative Problem Solving (CPS), Connect Model and Attachment Regulation and Competency (ARC).
- Role models and coaches YCW participation in therapeutic recreation and community activities with children and youth, as outlined in the care plan.
- Assist YCWs to implement practices of family centered care, trauma Informed care, and culturally informed care.
- Participates in milieu leadership team meetings to share feedback, strengths, and opportunities for improvement,
- Conducts home and school visits with YCWs and roles models best practice approaches while implementing strategies to improve skills and capacity of caregivers and educators, as per the direction from appropriate healthcare professional.
- Consults with the professionals overseeing case as required and appropriate
Hours of Work:
Monday to Friday, 8h00 - 16h00; must be flexible to accommodate early mornings and/or evenings as required
Your Qualifications:
- Successful completion of an undergraduate degree in Child and Youth Care Studies, OR an undergraduate degree and a graduate from a 2 year accredited Child and Youth Care diploma program
- Internal applicants currently working as Youth Care Workers with an undergraduate degree and a diploma from a 1 year accredited Child and Youth Care Program will be considered.
- Internal applicants currently working as Youth Care Worker I in the Mental Health and Addictions Program with 20 years' experience in child and adolescent mental health and addictions will be considered.
- CYC Practitioner certification preferred
- Minimum five (5) years' experience working with patients, families and interprofessional teams within a day treatment and inpatient mental health and addiction setting required.
