Position: Care Manager (Full-time, Contract until November 2025)
Working Hours: 35 hours per week
Reports to: Collaborative Practice and Care Services Manager

Position Summary: Responsible for coordinating client assessments, problem identification, service coordination, education, advocacy, discharge planning, and organizing support groups for clients and families.

Responsibilities:
Provide Care Management services
- Conduct assessments, prepare care/service plans, and manage care for clients
- Offer crisis management, counseling, and follow-up plans as needed
- Identify and document incidents, taking necessary actions
- Maintain accurate records in care plans, progress notes, and reports.
Organize and coordinate support groups and related services
- Arrange educational workshops in collaboration with other agencies or healthcare professionals
- Supervise volunteers supporting groups or individuals

Education:
- Post-secondary degree in a health-related field or equivalent experience
- Health professional with active membership in an Ontario regulatory body preferred
- Possess a valid First Aid and CPR certification (an advantage)
Experience & Skills:
- Two (2) years of experience with the senior population
- Ability to communicate in Tamil is beneficial
- Experience with older adults dealing with mental health and/or addictions is advantageous
- Proficiency in assessment, counseling, care management, and group work
- Strong interpersonal, communication, decision-making, and assessment skills
Other:
- Must undergo Vulnerable Sector screening
- Must have a valid driver's license and access to a car for home visits
- Occasional weekend and evening work might be required
