Description:

***: The Social Worker provides psycho-social assessments, individual and family interventions for the child/youth and family that are followed by in-patient, out-patient rehabilitation services as well as the neuromuscular and other clinic areas within Development and Rehabilitation.

The Social Worker will provide services on an individual and family basis for families and clients served by CHEO.

The social worker addresses and applies basic social work knowledge to the social, emotional, economic, cultural and environmental factors that may affect a client/family's ability to respond to a client's comprehensive health care needs in relation to their health care needs.


The Social Worker will provide counseling related to diagnosis and adjustment to illness, crisis intervention, and liaison with community-based agencies and services.

The Social Worker will advocate on behalf of patients and families with regards to community services/resources and financial assistance.

Social Work is provided in collaboration with the family, the multi-disciplinary team and the community.

The provision of the social work service will is based on evidence informed practice, using professional judgments and the integration of theory into practice supporting best outcomes for children and families.

Consistent with the inter-professional setting and the mission, vision and strategic directions of CHEO the Social Worker will be a member of the interprofessional team providing proactive care services at CHEO.


The social worker will participate in providing mentoring and training opportunities to social work students studying at the BSW/MSW level.


The social worker will be a member of the CHEO Discipline of Social Work and will participate in the Social Work Coverage Model.


RESPONSIBILITIES
***:

  • Provide psychosocial support and counseling to children/youth in Inpatient Rehabilitation, Outpatient Rehabilitation and Neuromuscular Clinic
  • Provide counselling and support to families by offering ongoing individual, family and group counselling, crisis intervention and/or conflict resolution related to being a caregiver.
  • Work closely with the interprofessional hospital and community teams
  • The Social Worker will also offer support and will provide liaison and coordination of community services/resources as required.
  • Work in various settings and environments in order to support parents and caregivers where they require interventions (home, school, work place, community organizations, etc.)
  • Conducts comprehensive psychosocial and family assessments by methods such as interviewing the patient and family, obtaining relevant information, gathering social data regarding the patient and family and formulation of assessment and plan of intervention, in accordance with professional practice standards and clinical policies.
  • Support other professionals who are working with families/caregivers to create awareness and build capacity within the larger support system network of lived experience and trauma informed care.
  • Maintains current knowledge on evidence based practice by such methods as reviewing literature, consulting with clinical and community experts in the field, evaluating clinical practice and participating in professional development activities in order to improve care through new practices.
  • Performs administrative tasks by maintaining clinical documentation, progress notes, workload statistics and updating health records in accordance with established standards.

QUALIFICATIONS Police Record Check (PRC) (Essential):

  • University Degree in Social Work (Essential)
  • Master's Degree (preferred)
  • Registered member of the Ontario College of Social Workers and Social Service Workers (Essential)
  • Demonstrated leadership skills. (Essential)
  • Demonstrated expertise and knowledge about the impact of chronic stress and complex illness on the family unit. (Essential)
  • Minimum 2 years' experience working with parent/caregivers in a hospital (Essential)
  • Demonstrated ability work independently. (Essential)
  • Demonstrated ability to conduct comprehensive psychosocial assessment. (Essential)
