Job Description

As a member of a 24-hr patient access service, the System Flow Coordinator is responsible for patient flow; to facilitate appropriate access to safe patient care. This will include coordination and accountability for decision making with respects to consultation, placement and movement of patients within and outside of tertiary acute care. The System Flow Coordinator will function as an integral member of an acute care team whose collective responsibility will include access to and movement through resources across the continuum, and to other health facilities. The System Flow Coordinator provides access to consultation with physicians, patients, families, nurses and support staff. During off-hours the System Flow Coordinator will function as a management representative with respect to problem solving and communication.

Job Qualifications

Required Qualifications

  • Baccalaureate degree (in progress) or Diploma in Nursing with Manager or leadership experience. Current licensure with the CRNS or RPNAS. Current CPR certification in Basic Life support at Level C. Knowledge of leadership theories and strategies in order to lead, encourage, advise and cooperate with colleagues to promote and maintain healthy, positive relationships while achieving client/patient, team and organizational goals. Knowledge of communication techniques and approaches in order to adapt to multiple and ever-changing situations and environments. Knowledge of critical thinking processes in order to creatively solve problems in an open and resourceful manner. Skillful in complex decision making through the application of knowledge, critical thinking and sound clinical, legal and ethical judgement. Exceptional interpersonal and communication skills. Knowledge of change theories in order to identify, promote and support change management. Knowledge of group processes, dynamics and organizational culture and how they contribute to the functioning of an organization.
Experience
Equivalent of 5 years recent experience in a medical, surgical, critical care or emergency department in an acute care facility is preferred. Previous coordinator or management experience is preferred. Intermediate computer skills with experience in Excel and Word.

  • Demonstrated clinical competency and sound decision making skills.
  • Demonstrated leadership with good organizational skills.
  • Demonstrates effective relationships with staff, supervisors, patients/clients, families, and physicians.
  • Exceptional communication skills.
  • Familiarity with consultation, admission, discharge and transfer of patients within the acute care system.
  • Familiarity with System Flow tools such as but not limited to APF, SCM and SER.
  • Understand consultation, admission, discharge and transfer of patients & clients to community partners.
  • Well developed analytical, judgement and decision-making skills.
Other Information
  • Employees will perform the work of more than one (1) former regional health authority from their home location.
  • Occasional Travel may be required.
