As a CareCoordinator , you will assess and determine patient care needs andeligibility, provide access and referrals to community services, and engagewith patients, caregivers and other health care practitioners.

Whether you work in ouroffice, in the community, or a health care facility – you will play a lead rolein providing connected, accessible, patient-centered care – and be supported byour collaborative team that includes over 8,000 regulated health care and otherprofessionals.

What will you do?

· In collaboration with patients and their families, assess careneeds, determine eligibility for services, and develop individual care andservice plans

· Linkpatients with service providers

· Coordinate and monitor care plan delivery

· Establish a helping relationship with patients and their families

· Balance patient needs and choices with available resources,ensuring patients' values and preferences are respected

What must youhave?

  • Membership, in good standing, with theapplicable regulatory body:

o College of Nurses of Ontario

o College of Physiotherapists of Ontario

o College of Occupational Therapists of Ontario

o Ontario College of Social Workers and SocialService Workers

  • Recent experiencein community health or a related field
  • Knowledge ofthe health care delivery system and community resources
  • Excellentinterpersonal, communication, assessment, problem-solving, anddecision-making skills
  • Effectivetime management, prioritization and organizational skills, with theability to work independently and co-operatively in a busymultidisciplinary environment
  • Establishedability to accurately complete required documentation, reports and forms
  • A validdriver's licence and access to a reliable vehicle
  • Advanced oraland written proficiency in English and in French is required
  • We have a mandatory COVID-19vaccination policy. As a condition of employment, all employees arerequired to submit proof of COVID-19 vaccination status prior to startdate.

What wouldgive you the edge?

· Experienceworking with diverse patient groups, e.g., multicultural, homeless, palliative,acquired brain injury (ABI), mental health, geriatrics, pediatrics

· Case managementexperience or recent related community experience

· Abilityto speak French or another second language

French Proficiency: French required
