The salary range for this position is CAD $42.CAD $52.Come work as a Mental Health & Addictions Case Coordinator with Vancouver Coastal Health (VCH) Vancouver Coastal Health is looking for a Mental Health Case Coordinator to join the Adult Mental Health team at Hope Center in North Vancouver, B.

As a Mental Health & Addictions Case Coordinator Nurse with VCH you will:

Work as part of a multidisciplinary mental health and addictions care team to provide treatment and case management services that are both inclusionary and flexible for individuals with mental illness and concurrent disorders (i.e. addictions.)

Perform key duties such as:

intake, mental health assessment, treatment planning, crisis intervention, consultation, individual counseling, education, and support groups to facilitate and coordinate client access to services.

Masters Degree in an Allied Health Discipline from a recognized program.

Three (3) years' experience providing therapy to adults with mental health issues, or an equivalent combination of education, training or experience.

Valid BC Drivers License. Local area travel may require the use of a personal vehicle.
Broad knowledge of other facilities and community resources.
Broad knowledge of other health care disciplines and their role in client care.
Broad knowledge of adult education principles, methods and tools.
Broad knowledge of legislation and government policies and procedures that relate to client.
Demonstrated ability to conduct mental health assessments, and provide treatment planning and crisis intervention.
Demonstrated ability to apply knowledge of theory and practice to a case management process.

Demonstrated ability to communicate effectively, both orally and in writing, with clients and their families, colleagues, physician, and other health care staff, both one-on-one and in groups.

Demonstrated ability to provide consultation, leadership and work direction.

Basic computer literacy to operate a computerized client care information system and word processing, spreadsheet, Internet and e-mail software.

As per the current Public Health Orders, as of October 5, 2023, all employees working for Vancouver Coastal Health must be fully vaccinated for COVID-19 or have received a single dose of the most-recent, updated COVID-19 vaccine.

Proof of vaccination status will be required.
