The North Simcoe Muskoka Specialized Geriatric Service (NSM SGS)

Program is currently seeking a permanent Full-time Psychogeriatric Resource Consultant (PRC).

This individual will work with health and social support providers and a wide range of community based organizations who serve older persons living with complexity and frailty with complex physical, cognitive, mental health and addiction needs.

PRCs advance clinical practice and build system capacity through the training, coaching and mentoring of health care providers; building clinical education programs; and supporting quality improvement work.

**Please note:
This position is located in the region of North Simcoe and South Georgian Bay.

Key Accountabilities:

  • Enable the uptake of evidenceinformed approaches to everyday practice through continual formal and informal education, coaching and mentoring within the SGS team and partner health and social service providers
  • Provide education as a qualified trainer in the SGS Education Program
  • Provide casebased consultation to health/social service providers to support the assessment, development, implementation and evaluation of clinical plans of care
  • Provide casebased consultation for the NSM SGS Complex Case Resolution Process
  • Facilitate linkages and partnerships between organizations providing health care, community and social services for older adults
  • Provide leadership to committees, networks and communities of practice that focus on older adults living with complexity and frailty; participate in committees, networks and communities of practice that work holistically to identify gaps and meet the needs of older adults and their care partners
  • Provides consultation, support and leadership in the development and implementation of: clinical programs, standards of practice, practice guidelines, educational strategies, quality management, and/or system change

Requirements/Qualifications:

  • Current regulated health care professional (registered with no restrictions) with a university degree, master's level preferred; Member of relevant professional associations
  • Three to five years recent and related working experience with older adults living with complexity and frailty
  • Certification in gerontology and/or geriatrics an asset
  • Recent training in: P.I.E.C.E.S, Gentle Persuasion Approach (GPA), Positive Approaches to Care (PAC) and Seniors Mental Health an asset
  • Experience in group facilitation, knowledgetopractice and adult learning theory in order to provide education, coaching and mentoring to providers and care partners
  • Experience in quality improvement theory and processes
  • Demonstrated leadership and interpersonal skills, including:
  • Demonstrated ability to engage others internal and external to the organization;
  • Demonstrated ability to empower teams and individuals in problemsolving, solutionfinding and conflict resolution;
  • Proven analytical and conceptual thinking skills in order to make decisions in response to complex problems and to manage risk
  • Working knowledge of computer software including proficiency in Microsoft Office Suite, database management
  • Results oriented work ethic to work independently and undertake tasks needed to accomplish work objectives and deliver quality, consistent and timely results
  • Proven written and verbal communication skills
  • Proven ability to adapt readily to change
  • Models and promotes core ethical practice, Waypoint Values, and reflects an optimistic and positive attitude
  • Preference for proficiency in French/English language skills
  • Valid Ontario drivers license, a safe and operational vehicle and an ability to travel distances across the NSM region
