We are currently looking to hire a:

Social Worker

- Part Time, permanent (21 hours per week)

Reporting To:
Manager - Community Health Team


Purpose:

The Hepatitis C (HCV) Program for Scarborough is in partnership with the Ministry of Health

Our goals are to:

  • Increasing overall access to HCV diagnosis and treatment.
  • To reduce the spread of HCV and reinfection.
  • To work with at risk populations to support education and health promoting behaviours.

Key areas of responsibility:

  • Ensures that appropriate treatment plans are developed, implemented and evaluated.
  • Monitors client progress includes evaluation of, and adjustments to, treatment plan, maintains supportive relationship with the client and family members, links to and coordinates required services while coordinating client care.
  • Participates as an active member of the multidisciplinary team in regular client conferences. The purpose of such conferences is to ensure regular communication between client and care team members regarding the needs of the client, provide a forum for discussion of possible resolutions to client care issues and to provide opportunity to initiate improvements in the quality of service provided to the client.
  • Acts as a clinical resource consultant to staff regarding care issues and participates with the multidisciplinary team in care planning.
  • Complete the Activities of Daily Living Index in the Disability Determination Package in accordance and with the regulation made under the Ontario Disability Support Program Act.
  • Participates on committees and working groups within the facility as requested.
  • Maintains current knowledge of community resources pertinent to the client base and practice area served.

Qualifications and Experience:

  • Master's Degree in Social Work (MSW).
  • Registration and a member in good standing Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Minimum 3 years of experience in adult mental health care team.
  • Experience in counseling or providing therapy with individuals and families from diverse communities and backgrounds.
  • Knowledge of policies, legislation, programs, and issues related to area of practice.
  • Experience working in community health and in an interprofessional model of health and wellbeing.
  • Critical Incident Stress Debriefing (basic and advanced).
  • Thorough knowledge and experience of HCV, harm reduction, addictions and mental health.
  • Excellent organization, communication and interpersonal skills and the ability to work collaboratively as a team member in a dynamic, interprofessional environment.
  • Sensitivity to and awareness of culturally, racially, economically and socially diverse groups.
  • Demonstrated ability to advocate for services to meet complex needs.
  • Demonstrated ability to provide Person and Family Centered Care.
  • Maintain accurate and concise confidential EMR documentation pertaining to client history and encounter data.
  • Must have demonstrated competence in clinical skills, including psychosocial assessments, counselling intervention, and ability to work within a multidisciplinary team approach.
  • Demonstrated expertise in various treatment modalities/approaches including but not limited to Cognitive Behavioural Therapy, Dialectical behavior therapy, Motivational Interviewing, Solution Focused, etc.
  • Must be able to communicate effectively verbally and in writing with professional and nonprofessional staff and clients.
  • Must have excellent organizational and professional skills.
  • Oral and/or written fluency in other languages relevant to providing service in the role would be considered an asset.
