About You:

You're known as the 'go to' person to talk to, given your warm and compassionate personality and how you make people feel heard.

You can quickly put yourself in someone else's shoes and relate to their unique challenges and needs. You have an impressive analytical mind that comes up with solutions to problems without getting frustrated.

Negotiation, diplomacy and tact are your trademarks, and you're known for always being so kind in your interactions with residents, families and team members.

Servant Leadership also comes naturally to you as you don't mind rolling up your sleeves to "jump in" when the situation calls for it.

You are obsessed with organization and preparation and always seem to find time in your calendar for unexpected situations that pop up occasionally.

Your years of experience in this field have made you very resilient, with an ability to be laser-focused on your resident's needs even when the situation is challenging.

You'll hold a Bachelor of Social Work degree and are eligible to be registered or are in good standing with the British Columbia College of Social Workers (BCCSW) MSW preferred.

You'll have previous experience in a long-term care environment and possess progressive and innovative approaches to gerontological service provision.

About The Role:

You'll be responsible for leading the provision of social services for our Inglewood Community residents and their families while promoting the physical, psychological, social and spiritual well-being of the seniors we serve.

As a multidisciplinary care team member, you'll advocate on behalf of the resident, significant other and team members as necessary.

  • You'll contribute to the Resident Care Plan by assessing the resident's psychosocial needs, facilitating resident care conferences, and assisting with resident and family counselling.
  • You'll consult the Director of Care and the nursing and recreation team regarding resident care and their service plan development.
  • You'll keep busy by staying uptodate on legislative programs and social services related to complex care and reviewing and revising policies and programs as needed.
  • You'll also maintain professional relationships with resident and community agencies, gathering information for bed matching and matching residents to available beds in consultation with the Director of Care/Administrator.
  • You'll also focus on reporting metrics by collating statistical data, establishing and reviewing quality improvement initiatives, and maintaining a private pay waitlist. The role is a fulltime, permanent position located in West Vancouver. You'll report directly to the Administrator.
