Your Opportunity:

The Outreach Worker is a member of the Corrections Transition Team Calgary (CTT), a multidisciplinary outreach team.

CTT Calgary works in an integrated and collaborative way to improve access and ensure the connection to quality, client-focused, culturally appropriate, and integrated addiction and mental health services for adults being released from provincial correctional institutions.

CTT Calgary provides a short-term transitional service of varying intensity.

As a member of CTT Calgary the Outreach Worker acts as a co-case manager with the clinicians on the team.

The Outreach Worker is responsible for assessing clients' level of daily function in major life areas and creating an outreach treatment plan in collaboration with the client and co-case manager to address basic, social, and recreational needs.

CTT Calgary typically does not do home visits or transport clients.

The Outreach Worker contributes to the successful transition of clients from correctional institutions to community by adding to a holistic treatment plan to strengthen community connections.

The Outreach Worker operates with a high degree of independence and demonstrates ethical practice and professionalism in working with staff, clients, peers, other health care providers and the public.

In addition, they have a working knowledge of relevant policies, acts and legislation including the Mental Health Act and the Criminal Code of Canada.


Description:


As an Outreach Worker, you will work as a member of a multidisciplinary team, supporting clients to stabilize in their current environments as an extension of their clinical care service.

As an Outreach Worker, you will conduct outreach assessments, determine outreach service goals and develop outreach treatment plans in collaboration with clients and clinicians.

You will facilitate client connections with community and government supports and advocate for clients with these external service providers. You will focus on establishing housing, financial, food and clothing, transportation and social supports for clients.

You will be responsible for providing education and coaching support related to daily living skills and activities (attending appointments, money management, searching for jobs, grocery shopping, etc.), observing and reporting changes in the clients' behaviours, participation and ability to function independently to the interdisciplinary team.

You will evaluate client progress and functioning and make adjustments to the outreach treatment plan as required.

As a part of your role as an Outreach Worker, you may be required to provide outreach crisis stabilization support for clients, exercising various de-escalation techniques and linking clients with the appropriate supports (EMS, Police and Crisis Team, etc.).

You also may be required to enter clients' residences and assist clients to transfer in and out of vehicles, care facilities and residences.

Required Qualifications:

Completion of diploma from a post-secondary program in a related field.

Additional Required Qualifications:

Minimum of 3 years' experience working with forensic clients who are experiencing mental health and addiction related problems. Demonstrated ability to work collaboratively in a multi-disciplinary team setting. Superior knowledge of relevant Calgary and area community resources (housing, income support, ID etc.) Excellent interpersonal skills. Superior written and verbal communication skills. Demonstrated critical thinking and clinical judgement skills. Ability to work confidently and independently within collaborative partnerships. Ability to thoroughly and accurately collect and document information. BCLS.

Preferred Qualifications:

Experience working in correctional settings or with corrections populations preferred. NVCI, Honos, LOCUS training an asset. Connect care training an asset.
