QUALIFICATIONS:

Two years in the past five years of working experience in an inpatient or outpatient medical setting.

Must possess therapeutic skills to conduct psychosocial interventions, crisis intervention and brief therapy interventions.
Knowledge of and experience in case management.
Skilled in counseling and educating individuals, families and groups regarding coping strategies and cognitive behavioural change theory related to lifestyle changes.
Knowledge of psychosocial and physiological impact of chronic illnesses and conditions, heart and stroke disease an asset.
Understanding and knowledge of available community resources.
Knowledge of all relevant legislation (i.e. Health Care Consent Act, Substitute Decision Act, Mental Health Act, Public Guardian and Trustee, Child Protection Act).
Demonstrated professionalism and community relations skills while dealing with the public (patients and families), as well as various medical and nonmedical professionals.
Evidence of effective customer service, organizational, interpersonal, communication, judgment, critical thinking and decisionmaking skills.
Ability to work effectively within a multidisciplinary team.
Flexible, selfdirected and able to work effectively with mínimal supervision.
Consistent and regular attendance required.
Proficient in both official languages (English & French) will be considered an asset.
Vaccines (COVID19 and others) are a new hire requirement of the job unless you have an exemption based on medical or on a ground pursuant to the Ontario Human Rights Code
