QUALIFICATIONS:
  • Bachelor of Social Work.
Two years in the past five years of working experience in an inpatient or outpatient medical setting.

  • Must possess therapeutic skills to conduct psychosocial interventions, crisis intervention and brief therapy interventions.
  • Knowledge of and experience in case management.
  • Skilled in counseling and educating individuals, families and groups regarding coping strategies and cognitive behavioural change theory related to lifestyle changes.
  • Knowledge of psychosocial and physiological impact of chronic illnesses and conditions, heart and stroke disease an asset.
  • Understanding and knowledge of available community resources.
  • Knowledge of all relevant legislation (i.e. Health Care Consent Act, Substitute Decision Act, Mental Health Act, Public Guardian and Trustee, Child Protection Act).
  • Demonstrated professionalism and community relations skills while dealing with the public (patients and families), as well as various medical and nonmedical professionals.
  • Evidence of effective customer service, organizational, interpersonal, communication, judgment, critical thinking and decisionmaking skills.
  • Ability to work effectively within a multidisciplinary team.
  • Flexible, selfdirected and able to work effectively with mínimal supervision.
  • Consistent and regular attendance required.
  • Proficient in both official languages (English & French) will be considered an asset.
  • Vaccines (COVID19 and others) are a new hire requirement of the job unless you have an exemption based on medical or on a ground pursuant to the Ontario Human Rights Code
