Position summary


As part of the Casework and Social Supports Team, the Health Services Caseworker will develop relationships with guests, connect with external harm reduction and health care partners, and direct guests to services as needed.

This role will offer connections and referrals to a broad range of services including primary health care, dental care, harm reduction and mental health supports.

We are committed to serving people who face barriers to healthcare and the Health Services Caseworker will focus on reducing these barriers.


RESPONSIBILITIES

  • Develop and implement guest focused health and harm reduction programming and projects;
  • Coordinate onsite health services with a focus on harm reduction;
  • Support the operation of the Overdose Prevention Space and provide health and harm reduction services as needed;
  • Facilitate transitions back to SFC, after returning from external services/agencies;
  • Facilitate appropriate transportation and accompany to and from external services/agencies;
  • Collect and analyze data related to harm reduction and health services with a goal of reducing emergency calls to respite site;
  • Collaborate with community partners to provide more onsite health services;
  • Enhance the capacity of internal and external stakeholders by providing timely feedback and coaching to support harm reduction and health promotion engagement with guests;
  • Complete comprehensive guest assessments to understand what services they are eligible for;
  • Assist guests in identifying health goals, ensuring goals are attainable and time specific, and that the guest plays an integral role in the decision making process;
  • Encourage the guest in problem solving, conflict resolution, harm reduction, and medication management as appropriate;
  • Facilitate and support SFC overdose prevention goals;
  • Model a trauma informed approach to health care and harm reduction practices on site;
  • Develop and maintain positive and collaborative relationships with stakeholders and community partners;
  • Collaborate with caseworkers as needed for coordinated guest access to required medical, clinical or other community resources, including housing, food, clothing, and other relevant services;
  • Work collaboratively with other departments to contribute to a positive and supportive work environment;
  • Participate in the development, implementation and evaluation of services provided;
  • Maintain accurate, uptodate records; complete and compile all necessary file notes, reports, statistics and other written documentation on a timely basis and in accordance with agency standards and Personal Health Information Protection Act;
  • Develop and implement goals and objectives that are outcome based and measurable, and which reflect the mission of the organization;
  • Contribute to the strategic and operational planning process of providing an alternative to relying on emergency services;
  • Conduct ongoing evaluation of health programs and services delivered;
  • Advocate, as required, on behalf of guests;
  • Provide or support the development of educational and awareness sessions, briefings, and resources pertaining to common issues observed while providing services;
  • Participate in continuous quality improvement initiatives;
  • Support the overall operations of the Centre;
  • Flexible to attend meetings scheduled outside of working hours as needed;
  • Ensure health and safety protocols and the use of personal protective equipment are maintained;

  • Ability to lift 50 pounds as required;

  • Safety precautions due to proximity with hazardous materials and/or contagious diseases;
  • Travel between locations and some scheduled overtime is associated with this position;

QUALIFICATIONS:

  • BSW, SSW, PSW, Health Promotion or another health related field of study or demonstrated experience in a related field;
  • Crisis Prevention and Intervention certificate;
  • CPR, First aid certificate;
  • Lived experience will be considered an asset;
  • Demonstrated reliability and availability to attend scheduled shifts;
