Mount Nemo Christian Nursing Home is a 60-bed long-term care facility located in north Burlington. Mount Nemo has a rich history and has been serving the community since 1985. The Home bases its care on Christian principles utilizing an interdisciplinary team approach.

About This Position:


The Social Service Worker is a resource for residents and families to assist with the transition into long-term care and navigating life in a communal living setting once here.

The SSW will work closely with the Life Enrichment Coordinator to receive referrals and develop interventions to assist residents with their unique needs.

All duties are carried out in accordance with the Fixing Long Term-Care Act, and Home policies and procedures. This position reports to the Life Enrichment Coordinator.

Duties:

  • Assist residents with the admission process and transition into longterm care
  • Receive referrals for SSW services, complete assessments and develop individualized interventions
  • Participate in resident care conferences, as applicable
  • Complete Mini Mental State Examination (MMSE), as requested by physicians
  • Educate staff on resources or alternative approaches to care and services
  • Locate and arrange resources that may be of benefit to residents
  • Complete documentation (progress notes, care plan updates, etc.)
  • Attend staff meetings and inservice educational programs
  • Comply with all provincial regulations and established department policies and procedures
  • Complete all other tasks as assigned by Life Enrichment Coordinator

This Role Gives you the Opportunity to:

  • Work in a smaller Home, allowing you to have increased focus on residents and families
  • Work closely in an interdisciplinary team, recognizing the value of all team members
  • Making a direct impact on resident lives

Qualifications:

  • College diploma in Social Service Worker from an accredited college
  • Current registration with the Ontario Association of Social Workers and Social Service Workers
  • Minimum 1 year experience working in a healthcare environment; experience working with an elderly population an asset
  • Ability to work both independently and as part of a team
  • Ability to read and understand English and be capable of following oral and written instructions thoroughly
  • Excellent customer service and interpersonal skills
  • Current (within 6 months) Vulnerable Sector Screening

Job Type:
Part-time

Part-time hours: 22.5 per week

Salary:
$24.72 per hour

Schedule:

  • 8 hour shift
