The Garry Hurvitz Centre for Community Mental Health at SickKids(GH-CCMH)is a child and youth mental health centre integrated withThe Hospital for Sick Children (SickKids).GH-CCMHis also a core service provider within the Toronto region of Child and Youth Community Mental Health Providers.

As such, the Centrehas an ambitious agenda to provide the highest quality evidence-informed care offering a range of services from prevention and early intervention through counselling and therapy, assessment and intensive supports.

Services are provided in both in-person and virtual formats.

This position is initially dedicated to the transition of children, youth and their families from inpatient units at SickKids into GH-CCMH community based mental health services and supports.

The position may evolve to include other transitional pathways from acute to community care.

This position will provide service planning, navigation and brief supportive counselling to children and youth experiencing mental health concerns, and their families, to ensure they move in, through and out of publicly funded mental health services in a timely and seamless manner.

As directed by, and in collaboration with Child and Family Therapist(s), provides clinical interventions and services for assigned children, youth and their families maintaining caseloads and supporting clients.

Participates in the implementation of individual treatment plans for clients assessing behaviours and responding to clients' needs.
Provides outreach and transitional support to clients, as required.
Coordinates cases with family/caregiver/relevant service providers/service planning team or creates materials for students relating to strategies/interventions.
Supports clients by facilitating social skills/therapeutic activities and organizing/attending events.
Prepares statistical data, as required.
Makes decisions related to day-to-day treatment issues and on-going intervention techniques.
Complies with the organization's vision, policies and procedures.

Post-secondary Diploma in Child and Youth Care and a minimum of one year of related experience or equivalent combination of education and experience; experience working in community is strongly preferred.

Member with the Ontario Child and Youth Care Association or equivalent association/accrediting organization.
First Aid/CPR, and TCI.

Good knowledge of MS Office Suites (Outlook, Word, Excel, PowerPoint), as well as ability to use office equipment efficiently (telephone, scanner, photocopier and fax).

Ability to engage in physical activities related to sports, board games, play based activities, etc., and UMAB training.
Ability to work flexible hours.
Valid driver's license and access to a vehicle is an asset.

Vaccination Requirements:
As a health care centre, our highest priority is the safety of patients, families and staff.

Therefore, any offer of employment is conditional upon proof of full vaccination with a Health Canada or World Health Organization (WHO) approved COVID-19 vaccination no later than fourteen (14) calendar days prior to the effective start date.

Should there be an exemption of vaccination based on a medical, or human rights reason, proof will need to be provided, in addition to participation in regular COVID-19 testing based on exposure risk as well as participation in vaccine education.
