POSITION:
Youth Concurrent Community Liaison

LOCATION:_In person to be Attended Daily; Location: Youth Centre's in Acton and Georgetown

REPORTS TO:_ Program Manager (Youth) or Clinical Supervisor (Youth)

This position will be classified as a Fixed Term Contract 0.6FTE (22.5hrs/week), Spread out across 4 days (3x5hrs and 1x7.5hrs)_

Delivers clinical concurrent disorders services to youth in Halton Hills by providing clinical assessments and developing and implementing clinically sound treatment plans; connecting clients to other community services as required; maintains client records; represents ADAPT with community groups, as requested.

ESSENTIAL DUTIES AND RESPONSIBILITIES_


The incumbent will fulfill these duties and responsibilities personally within the parameters outlined in ADAPT's Operations Manual, the Halton Region Community Investment Fund agreement, as well as the established standards of their professional regulatory bodies (if applicable).

Other duties may be assigned at the discretion of the Program Manager, Program Director or Chief Executive Officer.

  • Assesses client needs through the use of diagnostic tools and structured interviews
  • In consultation with the client, develops an individual treatment plan for the client
  • Provides individual and/or group counselling to address the client's needs
  • Provides clients with support specific to life skills, daily living skills, conflict resolution, crisis intervention and prevention, budgeting assistance, and community integration
  • Conducts group sessions as required
  • Refers clients to other appropriate support services and agencies according to the client's needs
  • Provides consultative services to community agencies, professionals or individuals as required
  • Conducts case conferences/consultation with treatment providers, including physicians, other community organizations and other significant person, as deemed appropriate
  • Advocates on behalf of clients
  • Maintains current information on community resources
  • Maintains a collaborative working relationship with Halton substance abuse and mental health agencies
  • Maintains accurate and complete clinical records and service tracking data
  • Provides all reporting as required by the Halton Region Community Investment Fund (HRCIF)
  • Provides in all reviews conducted by the Region of Halton pertaining to the HRCIF
  • Assists in the promotion of ADAPT and its programs to other agencies and the general public
  • Participates in meetings as required
  • Provides input into the operational and strategic planning related to ADAPT programs as required
  • Provides other services according to the conditions of the program funding
  • Participates in committees as required
  • Participates in clinical supervision as required
  • Participates in professional development activities as required
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the minimum knowledge, skills, and/or abilities required to enter the position.

EDUCATION

  • College diploma/ university degree in health or social sciences;
  • Addictions and Mental Health/ Concurrent Disorders postgraduate diploma is an asset;

LICENSES, CERTIFICATIONS AND CREDENTIALS

  • Registered Social Service Worker is an asset;
  • Nonviolent crisis intervention, first aid, CPR and ASIST certification are assets;
  • GAINS certification is an asset

RELATED WORK EXPERIENCE

  • 1 year of relevant experience, working with youth up to the age of 24;
  • Experience in:
  • Individual, family, and group therapy
  • Community development and networking
  • Record keeping and file management

SKILLS

  • Broad knowledge of addictions treatments and explanatory theories, mental illness, and community resources
  • Basic life skills counselling, supportive counselling, crisis intervention, advocacy, and group work
  • Managing clients with complex needs in varying stages of change
  • Strong communication and active listening is required
  • Ability to effectively present complex clinical i
