The Calgary West Central Primary Care Network (CWC PCN) is a non-profit organization committed to a healthier future. We work alongside more than 475 family doctors to provide support for continuous health care.


We are seeking a Patient Care Coordinator (PCC) to support physicians and the Patient's Medical Home (PMH) team members in the implementation of projects by focusing on the coordination of patient care to improve overall patient outcomes.


The PCC is an integral and active member of the PMH team, supporting physician members remotely using Electronic Medical Record (EMR) systems and digital health platforms.

The PCC works in conjunction with a Health Information Coordinator and with members of the healthcare team in a coordinated and collaborative effort to engage, encourage, and support patient health.


Responsibilities:

Panel management and EMR standardization:

  • Support routine panel maintenance and patient validation processes for ongoing panel management, such as confirming primary medical doctor, demographics, and validating patient information while performing outreach to patients.
  • Identify missing vitals, lab results, or screening, referrals to external organizations, etc.
  • Ensure results of recent tests and procedures are available to providers (i.e., information is available on Alberta Netcare).
  • Create and maintain accurate notes about PMH projects such as meeting minutes and ad hoc patient list requests.

Coordination of patient care:

  • Develop and use scripts to discuss the reason for a phone call and the reason for visit with the specified health professional.
  • Coordinate patient care with the appropriate PMH clinicians, including referral processing and ensuring patients are informed about any appointment information required.
  • Provide further instruction or information as required to patients prior to the visit.
  • Ensure notes from patient interactions are recorded in the EMR for physicians and clinic staff knowledge and to ensure open lines of communication between the PMH team members.
  • Support the PMH clinicians with scheduling using EMRs and digital health platforms.
  • Coordinate patient care by referral to the medical neighbourhood (i.e., patients identified in a PMH project that require care outside of the Patient's Medical Home)

Qualifications:

Education:

An Accredited Medical Office Assistant (MOA) certificate or a post-secondary diploma or certificate in a related area such as Licensed Practical Nurse, Medical Secretary, or Unit Clerk is required.

Equivalencies of education and experience may also be considered.

Experience:
A minimum of two years of experience in the following is required:

  • Strong administrative experience working within an interprofessional care team model.
  • Knowledge and experience with medical records management.
  • Ability to maintain filing systems and basic databases.
A minimum of two years of experience in the following is an asset:

  • Previous experience in handling confidential or sensitive information.
  • Healthcare experience, preferably in a primary care setting.

Functional knowledge:

  • Knowledge of medical terminology and medical records management is essential.
  • Knowledge of applicable data privacy laws is an asset.
  • Knowledge of and interest in primary health care and health promotion would be an asset.

Technical:
Microsoft Office (Word, Excel, Outlook, PowerPoint, Teams). Virtual patient care platforms. Electronic Medical Records (EMR) systems.

Background check:
A clear Criminal Background check is required to start.
