The UHN IC program caters to a range of specialty populations, requiring the provision of complex care and specialized nursing skills within the comfort of clients' homes. Tailored to support individuals discharged from UHN hospitals, nurses in this program deliver personalized nursing care that caters to each client's unique requirements. A valid driver's license and access to a vehicle are necessary for this visiting nurse position.

Skills required include a variety of clinical procedures such as pneumostat care, atrium dry seal care, T-tube care, IV Lasix administration, and other specialized medical interventions.

Additionally, services offered encompass wound care, intravenous (IV) therapy, administering injections, as well as the removal of PICC lines and chemo.

This diverse skill set reflects the nurses' ability to provide a high standard of care across different medical needs within a home care setting.

Assessment, Monitoring, and Care Planning/Coordination
Continuous evaluation of care plan interventions through ongoing reassessment and review of outcomes.

Ensuring patient care quality by identifying and addressing factors that impact practice autonomy and the delivery of high-quality care.

Enhancing client safety through the acquisition of knowledge and skills in line with our commitment to continuous quality improvement.

Reporting any client activities and pertinent concerns or changes regarding the client or care plan to the nursing supervisor and submitting relevant documentation to the case coordinator.

Possession of a valid First Aid or CPR Certification.
Proficiency in wound care or venipuncture is advantageous.
Experience in Acute Care and Medical-Surgical nursing is a plus.
Familiarity with community nursing is beneficial.
Dedication to continuous learning and professional development, utilizing critical thinking for client care provision.
Possession of a valid Ontario driver's license (G License) and a reliable vehicle for transportation is a plus.
