  • Promote Social Prescribing of the food program within Community Healthcaring KW's Primary
  • Care team and CHC programs.
  • 2. Grow connections and opportunities for food justice collaboration between the Food Market
  • Program and community partner agencies.
  • 3. Participate in team/staff meetings and committee work as assigned to support both service
- delivery and organizational goals.

  • 4. Support Food Program volunteers and help create meaningful volunteer opportunities.
  • 5. Facilitate foodrelated groups that focus on lifeskill development, capacity building, and
- social networking.

  • 6. Establish and maintain community food system resources for staff and program participants.
  • 7. Facilitate the gathering of food market demographic information and provide quarterly reports
- that will inform programs and services. Collaborate with the Health and Wellness team to develop,
- implement and evaluate CHC food
- programs and services.

  • 9. Develop and monitor operating budgets for assigned projects/programs and ensure expenditures
- remain within budget.

  • 10. Create and post social media content to promote the Food Market Program
  • 11. Work during both regular and extended hours (evenings/weekends) and in locations within
  • Kitchener to be determined by Community Healthcaring K-W.
  • 12. Work in a manner that ensures client safety and minimizes risk to clients, volunteers, and the
  • Centre.
  • 13. Work in a manner consistent with the Community Healthcaring K-W Vision, Mission, and
  • Guiding Principles Work in a manner that demonstrates selfreflection and personal
- accountability for work performance.

  • 14. Adhere to all Community Healthcaring K-W policies.
  • 15. Work from a Social Determinants of Health perspective to improve health outcomes for
- individual clients and/or for the development of community health and well-being, and to reduce
- health inequities.

  • 16. Work in a manner that preserves privacy and confidentiality.
  • 17. Work in a manner that ensures client safety and minimizes risk to clients, volunteers and the
  • Health Centre.
  • 18. Work in a manner consistent with the Health Centre's policy and commitment to Diversity,
  • Equity and Inclusion in all processes and practices.
  • 19. Work in a manner that demonstrates selfreflection and personal accountability for work
- performance

Qualifications:

  • 1. Desire to create accessible food programming and dedication to food equity.
  • 2. 2 years of work or volunteer experience within a notforprofit organization, particularly in
- food security systems.

  • 3. Familiarity with social service programs, health services, and systems in Waterloo Region
  • 4. Awareness of the Canadian Food Guide and the Region of Waterloo's healthy eating
- recommendations.

  • 5. Personal experience with food insecurity and/or awareness of issues faced by those experiencing
- food insecurity.

  • 6. Experience in program planning, group facilitation, and evaluation
  • 7. Strong crisis intervention skills
  • 8. Proven skills working with volunteers.
  • 9. Ability to lift 30 lbs.
  • 10. Experience supporting culturally and economically diverse populations.
  • 11. Ability to effectively multitask in a fastpaced environment, take initiative and work
- independently with mínimal supervision.

  • 12. Ability to work collaboratively as a member of a team.
  • 13. Ability to communicate effectively in English, both written and verbally.
  • 14. Excellent interpersonal skills to interact and engage effectively with individuals, community
- partners and groups.

  • 15. Ability to maintain strict confidentiality.
  • 16. Valid driver's license, access to a personal vehicle and minimum $1,000,000 liability insurance.
  • 17. Ability to obtain a Police Vulnerable Sector Check satisfactory to the Health Centre.
  • 18. The Health Centre requires that all staff be fully vaccinated for Covid19 at the time of hire
- and maintain vaccination ongoing as boosters become available. Official proof of Covid-1- vaccination status is a mandatory requirement
