Reporting to the Manager, Collaborative Practice and Education, the Social Worker may be assigned to various programs within the Health Centre depending on need.

The successful incumbent may be called upon to provide backfill for vacation, sick time or extended leaves of absence Monday through Friday.


Duties and Responsibilities:

  • The role of the Social Worker is assess the psychosocial needs of patients and their families and to facilitate discharge planning, education and community referrals
  • This position may include coverage to surgical, medical, critical care, Family Birthing, paediatrics, palliative care and/or mental health units
  • Able to work effectively as a member of an interprofessional team, collaborating with other disciplines to provide comprehensive patient care
  • Provides patient care in accordance with all professional and organizational standards

Qualifications:

  • Master of Social Work Degree required.
  • Member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW), with no terms, conditions, or limitations required
  • 23 years recent experience in Social Work Practice in a hospital environment required
  • Demonstrated knowledge and skills in psychosocial assessment, conflict resolution, creative problemsolving and complex discharge planning to other facilities (e.g. longterm care)
  • Demonstrated experience with the geriatric population and general medicine patients.
  • Working knowledge of community resources that facilitate patient discharge.
  • Strong communication and interpersonal skills and the ability to negotiate with patients/families, interprofessional team members, and other stakeholders.
  • Excellent organizational and coordination skills and the ability to prioritize multiple competing workload demands.
  • Ability to work cooperatively and constructively with other team members and develop effective partnerships with community agencies/facilities.
  • Demonstrated flexibility and ability to work under pressure in a fast paced environment.
  • Flexible to meet additional responsibilities as program demands
  • Must have availability Mon to Fri.
  • Satisfactory attendance.
