JOB SUMMARY:

Home and Community Care Worker is an integral member of our Home and Community Care team. In this role you support Tsleil-Waututh Nation community members by maintaining their ability to continue living at home safely. A Home and Community Care Worker assists with clients' personal care, monitors health changes, performs various housekeeping tasks, meal preparation, grocery shopping and companionship activities for clients in their homes.

DUTIES & RESPONSIBILITIES:

Assist with personal care as per care plan including but not limited to:

  • Bathing, toileting, oral care, skin care, hair care, dressing, nail care
  • Feeding and mobilization
  • Rehabilitation exercises
  • Medication reminders
  • Health advocacy
  • Observe, document, and report changes in the client's physical condition, behavior or appearance

Assist with the activities of daily living including but not limited to:

  • Light Housekeeping
  • including but not limited to vacuuming, dusting, dry mopping, dishwashing, cleaning the kitchen and bathroom, and changing beds
  • Laundry and linen service to include washing, ironing and folding clothes
  • Prepare meals and snacks according to care plan this may include following a specialized diet plan as identified on the care plan
  • Assist with shopping for grocery and clothes
  • Escort clients on appointments, including doctor visits, friends, family members, or stores
  • Monitor client supplies
  • Provide companionship for the client. This includes accompanying client on walks, trips to social/recreational activities, assistance with hobbies, etc.

Assist the Home and Community Care team:

  • Participate in team and department meetings
  • Provide support to the Public Health and Home and Community Care RNs, i.e. appointment bookings
  • Assist in organizing community health events, i.e. mass vaccination clinics, health and wellness fairs
  • Collaborate with external health care and social services partners

QUALIFICATIONS/REQUIREMENTS:

  • Health Care Assistant Certificate preferred
  • Grade 10 or equivalent plus a minimum of one (1) year of related experience in house cleaning, maintaining healthy environments, direct home support program delivery
  • May consider alternate combinations of relevant education and work experience.
  • CPR, firstaid and food safe certifications
  • TB screening
  • A valid BC driver's license and reliable transportation is an asset
  • Must successfully pass a Criminal Record Check
  • Must be fully vaccinated against COVID19 (minimum of 2 doses)
  • Previous experience working in a First Nations community and/or organization is preferred
