The Ontario Palliative Care Network (OPCN) is an organized partnership of community stakeholders, health service providers and health systems planners. Together, the OPCN develops a coordinated, standardized approach to high-quality, sustainable and person-centred hospice palliative care for all Ontarians, regardless of age or disease type. The OPCN Secretariat, a part of Ontario Health, executes the mandate of the OPCN, and supports the operational and tactical activities of the OPCN. The Specialist works with the OPCN Secretariat team to support the implementation of key areas in the Action Plan and work on the Compassionate Care Act. The Specialist contributes to project planning and execution by conducting planning, research and analysis activities.

They will:

Identify and leverage best practice models that integrate palliative care training and education for interdisciplinary teams and others (support staff, volunteers, caregivers, etc.) to develop resources, tools and supports to enable quality improvement, including communication skills, advocacy training, and culturally sensitive care as core competencies.

Develop and implement provincial education strategy, leveraging existing provincially and nationally endorsed education and training resources and where necessary develop additional tools.

Support opportunities to spread and scale existing work and resources developed by OPCN and in OH regions

Where Indigenous communities wish to be involved, provide support to Indigenous communities and Indigenous organizations for their implementation efforts.

Here is what you will be doing:

Support the managers, team leads and other team members in the development and maintenance of work plans to ensure completion of deliverables on time, and escalate project risks as appropriate

Develop and maintain internal OH relationships and regional team relationships to advance regional priorities and project goals

Develop various products including but not limited to briefing notes, analytical reports, executive summaries, status updates, and presentations to support internal and external communications, business reporting and decision-making

Work closely with local clinical experts and other stakeholders to identify opportunities and organize palliative care education

Define and implement the required frameworks, methodologies and standards within a specific area of expertise

Apply health care system knowledge and health literacy best practices to support the design, development and implementation of education resources

Apply an equity lens to the development of education strategy work and engagements

Maintain contact lists, orientation materials and support agenda development and material preparation for committees, working groups, etc.

Here is what you will need to be successful:

Education and Experience

Bachelor's degree in Health Science, Health Administration, Public Health, Business Administration, Health Informatics or related discipline, or equivalent education/experience

Master's Degree in one of the above disciplines is an asset

Minimum three years of overall working experience, with a minimum of two years recent experience working within the healthcare sector

Certification and/or knowledge/skills in change management, quality improvement, implementation science, knowledge translation is an asset

Experience with palliative care projects is an asset

Experience with community health organizations or long-term care homes is an asset

Knowledge and Skills

A commitment to developing a coordinated and standardized approach for delivering palliative care services for all people in Ontario, regardless of their age or illness

An understanding of Ontario's health care system and key provincial priorities for palliative care and broader health system improvement, with knowledge of about health care delivery and especially in primary care, homecare and long-term care

Expertise in using data from qualitative and quantitative analyses and other sources to advance QI including supporting completion of literature reviews, jurisdictional scans and other data gathering/collection activities

Experience in analyzing, synthesizing and summarizing information into summaries reports, briefs, business cases and other documents

Effective inter-personal skills and ability to work with diverse stakeholders

Solid communication skills, both oral and written. Internally, communicates with colleagues throughout the organization to discuss information and problems related to program implementation. Externally, communicates directly with various stakeholders together with manager and/or director when required

Awareness and understanding of QI and project management principles and approaches

Able to function independently in managing assigned work including identifying potential risks/issues as they arise, with manager support

Good problem solving, critical thinking and analytic skills to support effective decision-making and risk/issue management

Able to excel in a fast-paced and objective-based environment

Very good attention to detail with good organization and time management skills

Supports the advancement of equity, inclusion, and diversity across Ontario's health care system

Supportive in engaging with First Nations, Indigenous, Metis and urban Indigenous people as they self-determine their own approach to palliative care

Advanced knowledge of Microsoft Outlook, Word, Teams, PowerPoint, and Excel
