Horizon Health Network (Horizon) is seeking a strategic, innovative, accomplished, and detail-oriented individual to join our team as a Collaborative Care Seniors Health (CCSH) Clinical Leadership Manager. The ideal candidate will have experience leading priority projects, helping to form collaborations, building knowledge translation capacity, and building clinical leadership capacity.

Who you are

You have a worked in a healthcare related environment or within a senior care environment for a minimum of three to five years. You have experience in older adult clinical care, knowledge translation, project management, lean six sigma, staff education, policy development, change management, and stakeholder relationship building.

You have:

  • An advanced proficiency level in Microsoft Office applications.
  • Excellent organizational and time management skills
  • Excellent facilitation skills
  • Effective interpersonal, verbal and written communication skills
  • Demonstrated experience in working within a multidisciplinary team
  • Ability to travel throughout the province as required
  • High regard for confidentiality

Degree in science or degree in health-related discipline with appropriate licensure. Equivalent combination of education and experience may be considered.

You have a worked in a healthcare related environment or within a senior care environment for a minimum of three to five years.

Written and spoken competence in English is required at an advanced level.

What you will do

  • Compile, review and analyse best-practises involving seniors to support specific projects or requests.
  • Represent and promote the objectives and activities of the CCSH Clinical Leadership Task Force to various internal and/or external constituencies, as appropriate.
  • Build strong supportive relationships among diverse stakeholders, creating and using effective communications and knowledge translation tools with different end-user groups.
  • Develop mechanisms to ensure best-practice and standardized clinical practices are being used consistently across the province of New Brunswick.
  • Analyse and recommend strategies in knowledge translation and communications that further the reach and implementation of CCSH policies and guidelines.
  • Participate in the development of strategic and operational level planning for the CCSH by being an integral member of the CCSH Clinical Leadership Force.
  • Provide knowledge expertise and act as a resource person for CCSH clinicians and CCSH collaborators.
  • Support the writing of grant proposals and the dissemination of research findings through presentations.
  • Supervise staff who are participating in projects conducted by CCSH.
  • Act as a facilitator to guide working groups through the completion of priority projects.
