JOB PURPOSE

To provide advanced clinical nursing expertise in the assessment, planning, implementation, and evaluation of care to patients within the Internal Medicine Program. This role oversees management of patient care, working closely with the multidisciplinary team to develop and implement an individualized plan of care through assessment of the patient's physical, psychosocial, and educational needs. In addition, the Nurse Associate acts as a resource for nursing staff and allied health professionals.

QUALIFICATIONS

  • Graduate of an approved school of nursing and Bachelor of Nursing degree.
  • Current RN registration in the Province of New Brunswick or registration eligibility.
  • Minimum of 5 years current experience in in Internal Medicine nursing or equivalent critical care experience.
  • Successful completion of Critical Care Course is an asset.
  • Current certification in BCLS, Venipuncture, and Initiation of IV Lines
  • Current certification in ACLS, Defibrillation or willingness to obtain.
  • Demonstrates competency as a practitioner with demonstrated accuracy and attention to detail which is an essential element in this competency.
  • Demonstrates problem solving and critical thinking skills.
  • Demonstrates advanced skills in physical assessment and interpretation of clinical data.
  • Proven leadership qualities as demonstrated by initiative, good judgment, and the ability to instill confidence in peers.
  • Demonstrates the able to synthesize objective and subjective clinical data and document/communicate in a sequential and logical manner.
  • Ability to work independently and as a part of a multidisciplinary team.

KEY RESPONSIBILITIES

Under the supervision of the service physicians, the Nurse Associate:

  • Collects admission data and documents findings on patient record. Dictates discharge summary for the chart (to be reviewed and signed by the attending physician).
  • Assesses patients daily as their condition dictates.
  • Reviews clinical findings and any concerns identified with attending physician and initiates interventions and referrals within established protocols.
  • Participates in inpatient ward rounds as needed.
  • Performs advanced nursing competencies according to approved hospital protocol.

May include but is not restricted to:

  • Removal of chest tubes
  • Insertion/removal of feeding tubes
  • Removal of drains.
  • Removal of central lines.
  • Initiates specific medications and treatments under the supervision and with the knowledge of the attending physician or designate, according to predetermined and documented protocols.
  • Monitoring, management, and delivery of treatments/infusions for the patient in the clinical environment as needed.
  • Initiates orders, ICU transfer orders, establishes written discharge orders as needed by following established protocols.
  • Initiates consults to other services according to pre-determined documented protocols and/or physician direction. Assesses and collects data on consults to service.
  • Liaises with the inpatient unit Charge Nurse to develop and implement an individualized plan of care for patients identified to be at a greater risk for negative outcome or having barriers to discharge.
  • Assists patients in developing a discharge plan.
  • Manages patient expectations by providing education with a focus on treatment and the patient's role in recovery.
  • Collaborates with members of the multidisciplinary team to provide effective and efficient care and service.
  • Participates in the development and implementation of standards for nursing practice, RN and LPN, Advanced competency protocols, and for patient care protocols.
  • Takes a leading role in the identification, development/revision, and delivery of patient/family teaching materials.
  • Participates in general teaching sessions of the Department of Internal Medicine.
  • May participate in ongoing research projects.
  • Participates in meetings/discussions and reports on QI initiatives.
  • Provides leadership in the development of revision of research based clinical protocols and guidelines.
  • Identifies the need for change related to clinical, educational or research issues in clinical practice.
  • Maintains competency in the skills and knowledge necessary for advance practice nursing.
  • Acts as a role model to nursing staff to promote professional development, critical analysis skills and refinement of clinical practice.
  • Fills an educational role involving teaching nurses, physicians-in-training, individuals from related disciplines and family members.
  • Presents relevant information and the results and implications of research at rounds, workshops, and conferences.
  • Provides leadership in the development of revision of research based clinical protocols and guidelines.
  • Evaluates and implements clinical research findings to improve patient care in collaboration with appropriate medical and/or nursing personnel.
  • Participates in nursing or interdisciplinary research.
  • Clinical decisions are made in conjunction with the physician and health care team using department and hospital policies and procedures and expanded knowledge base.
