Community Access Occupational Therapist – Adults, Phys Med & Geriatric Health (Scarborough Region)
Monday to Friday, 8-hour shift
Full-time Salary Employee
VHA's Community Access Occupational Therapist is part of an innovative model of client care contributing to the rapid increase in home care services for the aging population. This is a key team member to ensure Ontario Health atHome waitlisted clients have access to OT services in a timely manner. This role also provides administrative and scheduling support so you can focus on direct client care. #VHA's Community Access Occupational Therapists have the following key areas of accountability, but not limited to:
Assesses and implements Occupational Therapy Interventions in a community setting in accordance with professional practice guidelines and standards established by VHA and the perspective regulatory college.
Assesses clients using interviews, standardized and informal assessment techniques in accordance with professional practice guidelines and standards established by VHA Rehab Solutions.
Interventions include but are not limited to: Home safety equipment and mobility aids, falls preventions and energy conservation strategies, palliative care, ADL Training, cognitive remediation/compensatory strategies, and linking to community resources/funding.
Degree in Occupational Therapy from a recognized school of Occupational Therapy.
Experience within the professional discipline is an asset.
Demonstrated ability to recognize safety concerns, communicate effectively using a computer and/or mobile device
