Are you an experienced registered nurse seeking a rewarding career that caresfor others, in a professional practice that cares for you? As a Care Coordinator , you will assess anddetermine patient care needs and eligibility, provide access and referrals tocommunity services, and engage with patients, caregivers and other health carepractitioners.
Whether you work in our office, in the community, ora health care facility – you will play a lead role in providing connected,accessible, patient-centred care – and be supported by our collaborative teamthat includes over 8,000 regulated health care and other professionals.
In collaboration with patients and their families, assess careneeds, determine eligibility for services, and develop individual care andservice plans
· Coordinate and monitor care plan delivery
· Establish a helping relationship with patients and their families
· Membership, in good standing, with theapplicable regulatory body:

Recentexperience in community health or a related field
Knowledge ofthe health care delivery system and community resources
Effectivetime management, prioritization and organizational skills, with theability to work independently and co-operatively in a busymultidisciplinary environment
Advancedoral and written proficiency in English and in French is essential.

· Casemanagement experience or recent related community experience
