POSITION OVERVIEWThe Palliative Clinical Coach is responsible for driving practice change, with a focus on improving access to and quality of clinical care, among participating community organizations (ie: primary care, community health centres, retirement homes, etc.). This position has a functional relationship with the KW4 OHT leadership for guidance and implementation directions. The work will be in alignment with the provincial directions established by Ontario Health with guidance developed by the Ontario Palliative Care Network, the Palliative Care Health Services Delivery Framework in the Community (“The Delivery Framework”), and the Ontario Palliative Care Competency Framework.The Palliative Clinical Coach will provide palliative care coaching and mentoring to build primary-level palliative care competencies; collaborate with leadership in community organizations on service integration and change management initiatives; consult on clinical care as required; and, on request, provide support to Indigenous communities and organizations collaborating with regional partners.
