POSITION OVERVIEWThe Palliative Clinical Coach is responsible for driving practice change, with a focus on improving access to and quality of clinical care, among participating community organizations (ie: primary care, community health centres, retirement homes, etc.). This position has a functional relationship with the KW4 OHT leadership for guidance and implementation directions. The work will be in alignment with the provincial directions established by Ontario Health with guidance developed by the Ontario Palliative Care Network, the Palliative Care Health Services Delivery Framework in the Community (“The Delivery Framework”), and the Ontario Palliative Care Competency Framework.The Palliative Clinical Coach will provide palliative care coaching and mentoring to build primary-level palliative care competencies; collaborate with leadership in community organizations on service integration and change management initiatives; consult on clinical care as required; and, on request, provide support to Indigenous communities and organizations collaborating with regional partners. Graduate degree (MScN, MSW) an assetMinimum 5 years experience in the practice of palliative carePrevious experience in clinical supervision and management of programs and staff preferredPrevious experience in education, research, project management, and/or quality improvement initiatives an assetDetailed understanding of health care system legislation and key organizations including Ontario Health, OH Teams, Home & Community Care Support Services (H&CCSS), acute care hospitals, community care, long term care, primary care and community support servicesSKILLS AND ATTRIBUTESExceptional communication skills in providing detailed explanations and interpretation while using courtesy, diplomacy, tact, and discretionAdvanced assessment and clinical reasoning skillsExcellent planning, organizational and project management skillsCommitment to equity, diversity and inclusion with an understanding of First Nations, Indigenous, and Metis cultural practices and approach to palliative careSelf-motivated, very organized and sensitive to the needs of clinicians and staffAbility to adapt to changing requirements and environmentAbility to manage time constraints and quality pressures Technically proficient with MS Office products and specialty software (such as D2L Brightspace)Access to reliable transportation and regular travel within the KW4 OHT geography is a requirement
