Job Title: Nurse PractitionerLocations: Oshawa, OntarioEmployment Status: Temporary Full-Time Contract Working within an integrated team, the successful Nurse Practitioner will provide comprehensive, client-centered primary healthcare that emphasizes accessibility, health promotion, illness prevention, and continuity of care for clients residing in high priority neighborhoods through a mobile community-based model.The NP will provide primary care assessments related to screening, prevention, treatment, and referrals to community programs. The NP will activate clinical pathways, referring clients to additional services, and connecting them with community-based and specialty services as indicated.DUTIES & RESPONSIBILITIESThe responsibilities of this role include, but are not limited to:Within a community mobile outreach model, perform clinical assessments, and therapeutic interventions within full scope of practice to address the biopsychosocial needs of clients, including those with multiple comorbid health conditions, mental health diagnoses and/or concurrent disorders involving substance use/abuse, recognizing the importance of social determinants and associated health risks.Participate in client care through a community mobile clinic, and perform clinical assessments combined with sound clinical judgement/diagnostic and treatment skills. Complete timely and accurate medical records of client encounters, documentation and third-party correspondence using the Electronic Medical Record (EMR) system Contribute to activities related to the development, implementation, and evaluation of medical directives, policies and procedures, and best-practice guidelines. CANDIDATES MUST HAVE:A current and valid Nurse Practitioner license with the College of Nurses of Ontario (CNO).Completion of a Master’s degree in Nursing.Current and valid CPR certification.Minimum of three years’ experience working as a primary care Nurse Practitioner with adult, children and youth with complex needs experience is required. Experience working in a community health and with individuals who face barriers accessing traditional health care services is an asset.Flexibility to work in Oshawa, Whitby, Ajax or Pickering as needed. Valid Ontario Driver's license and access to a reliable vehicle.Demonstrated ability to communicate with clients in a non-judgmental manner that can be effectively understood – includes active listening skills to understand client needs and coaching of clients as collaborative partners in their health care. ** Only candidates selected to be moved forward in the recruitment process will be contacted by Bayshore’s Recruitment Team to schedule a conversation.
